The following was sent to the person in charge: Dear Ms. Payton, The following items are needed to facilitate health department approval of the plans submitted for Pizza Hut to be located at 4701 Plank Road, Ste #103, Fredericksburg, VA 22407: With regards to the Food Establishment Permit Application: · Please fill out all highlighted areas that were left blank and change the answers to #1 to yes and the seating capacity to the appropriate amount. With regards to the Foodservice Plan Review: · Please fill in all unanswered areas that are highlighted throughout the application package. · Please provide a menu for the establishment and food suppliers that will be used. · Under cold storage: provide accurate number of refrigeration units including prep cooler, drinks cooler, etc. · Under Preparation: #4 stated there is an existing policy that will stay the same, please provide a copy of this policy. · Under Preparation: #5 stated that the chemical type being used to sanitize equipment (such as counter tops, cooking equipment, etc) that cannot be placed in a dishmachine or 3 compartment sink is called “Ecolab” and no test kit will be provided. Please provide the type of sanitizer (such as quaternary ammonium, bleach, etc.) and ensure a test kit to monitor this concentration will be provided. · Under Plumbing Connections: The plans stated that an ice machine and walk in cooler/freezer will be provided. Please indicate the plumbing connections for these areas.
