    Observation: The interior surfaces of the mechanical warewashing machine are soiled with limescale that may decrease the effectiveness of the unit.
    Correction: Increase the frequency of cleaning or limescale deposit removal from the interior surfaces of the unit to maintain unit performance.
  • Equipment - Non-Food Contact Surfaces and Utensils
    Observation: Surfaces of the ice machine was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: Insect control device is located over exposed food in kitchen where dead insects may be impelled or fall.

    Observation: The prepared ready-to-eat (RTE) foods in the refrigeration unit is not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: There was no temperature measuring device located in the low boy refrigeration unit.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose (corrected on site)
    Observation: The handwash station at the kitchen is being used for purposes other than washing hands.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Mops - Drying Mops (corrected on site) (repeated violation)
    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
  • Critical: Toxics - Identifying Toxic Containers* (corrected on site)
    Observation: Chemical spray bottle observed without a label (glass cleaner).
    Correction: Label spray bottles with contents or discard.

Follow-up inspection to Pre-Opening inspection on 12/6/12. The following violations were corrected:
1. Mops hung up to air dry.
2. Employee hand wash signs placed throughout.
3. Single-service items covered & protected.
4. Can opener blade replaced.
5. Sanitizer test strips available for 3 compartment sink.
6. Seal around drains applied.
7. Ice machine was cleaned.
8. Microwave was cleaned and sanitized.
9. Thermometers were placed in all units.
10. Backflow prevention was placed on mop sink.
11. Door gasket replaced on reach-in refrigerator.
12. No smoking sign was placed at entrance of establishment.
Employee health policy was signed by all employees (including English and Vietnamese). Permit was issued, 30 day inspection will be conducted upon opening.

No violation noted during this evaluation.

Permit not issued. Discussed posting of food additive instructions for staffing, proper utensil storage. Discussed employee health policy, glove usage. All observations shall be corrected prior to permit issuance. Hot water capacity appears sufficient but not tested at peak demand. Documentation provided by contractor utilizing different data calculation states sufficient hot water demand. Operator understands if hot water demand is not met during peak usage additional hot water equipment will be needed to meet compliance.
  • Wood, Use Limitation
    Observation: A wooden lid/food grinder is used as a food contact surface.
    Correction: Discontinue use of wood or wood wicker for this purpose. All food contact surfaces must be smooth, durable, and nonabsorbent, to prevent deterioration and to maintain an easily cleanable surface. Wood, if used, shall be hard maple or an equivalently hard, close-grained wood.
  • Temperature Measuring Devices

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonia test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The can opener blade is rusted and dull.
    Correction: Disassemble the can opener, clean and sanitize the assembly, remove and replace the blade. Can opener blades must be kept sharp to prevent creation of metal fragments that can contaminate food in the can when opened.
  • Equipment and Utensils - Good Repair and Calibration
    Observation: Reach in refrigerator gasket was observed in a state of repair and condition preventing effective maintenance and easy cleaning.
    Correction: Discard, replace or repair the gasket to provide proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: interior of ice machine
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Equipment - Cooking and Baking Equipment
    Observation: The cavity of the microwave oven is observed soiled.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.
  • Equipment, Utensils, Linens, and Single-Service and Single-Use Articles (corrected on site)
    Observation: Clean utensils were observed stored uncovered.
    Correction: Store equipment and utensils covered or inverted to prevent contamination while in storage.
  • Kitchenware and Tableware
    Observation: Single-service or single-use articles not handled, displayed, or dispensed so that the food- or lip-contact surfaces are protected from contamination (Straws,single service tableware)
    Correction: Handle, display, or dispense single-service and single-use articles so the food- or lip-contact surface are protected from contamination.
  • Critical: Backflow Prevention Device, When Required*
    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin. (Mop sink)
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Floor or floor covering around pip under three comaprtment sink is not smooth and easily cleanable.
    Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.
  • Handwashing Signage/Handwashing Facilities
    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.
