The purpose of this visit was to determine the door that separates the smoking room and restaurant was in compliance. EHS and EHS supervisor evaluated the door and concluded that the door was properly sealed and separated. Facility is now in compliance.
No violation noted during this evaluation.
05/09/2014Follow-up
Today's visit was to conduct a follow-up inspection on the door that separates the bar and restaurant. Although the door is better sealed, it could not be determined at the time of inspection that the door is sufficient as a barrier between the bar and restaurant. Another follow-up is scheduled within 10 days or less to determine the status of the door.

    Observation: Dead roaches were found on the premises.
    Correction: Remove dead rodents, birds, and insects from the premises to ensure that the establishment is clean and sanitary and to prevent carcasses from attracting other pests.

    Observation: Observed that floors under and behind equipment is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
04/10/2014Routine
The purpose of this visit was to conduct a routine inspection in conjunction with a follow-up meeting. On April 2, 2014 extension was granted and a meeting was setup to discuss with the CFM and the owner for April 10, 2014. During today's meeting, EHS discussed with owner and CFM with suggestions to make sure the door between the bar and eating area is properly sealed (meaning no smoke should be entering into the eating area). EHS also recommended to use a professional contractor to work with the door. This will be the last extension given. A follow-up inspection will be done on or about April 29, 2014. If the door is not properly sealed during the next follow-up inspection, then enforcement action may occur. Please contact EHS if door is completed before the deadline or if you have any questions.

The purpose of this visit was to follow-up on the door between the smoking and non-smoking area was properly sealed. During today's observations, EHS saw some type of rubber coating that was fitted around the door but it was still not sealed properly. Another follow-up inspection will be done for the door to be properly sealed. If the door is still not fixed, then enforcement action may be warranted.

EHS observed door between the dining area and smoking area still not sealed properly. EHS will follow-up again in 1 week to check that the door will be properly sealed. 03/20/2014Other
The purpose of this visit was to conduct a complaint investigation. Complainant allegedly saw customers smoking in the dining area which is non-smoking. CFM was not aware of the complaint and no other complaints were known. EHS observed no smoking signs posted in the dining area. No smoking paraphernalia was observed. Although a faint smoke smell was detected (by entrance), it could not be determined at the time what the cause could be. Swinging door between bar and dining room was closed. CFM does not know if there is a separate ventilation system. Bar is smoking area and is separate from dining area. Gaps were noted on the door and need to be sealed. EHS reminded CFM that smoking should not be allowed in the non-smoking section. Based on today's observation, complaint is not confirmed.

Today’s visit was to conduct a Risk Factor Assessment inspection.
**The employee health policy information was given to the facility at the June 18, 2013, June 14, 2012, and December 28, 2011 inspections. However, an effective employee health policy has not been established. The information was left with the certified food manager at this inspection
