Observation: In-use utensils improperly stored between use. Observed scoops stored in food items in prep cooler with handle in food.
Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

Observation: Sliced tomatoes (40), pepperoni (47), and mozerella cheese (50) cold holding at improper temperatures
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Items were relocated to walk in cooler.

Observation: Plumbing connections under the handsink in the rear of facility are leaking.
Correction: Plumbing systems and components shall be maintained in good repair.

Abbreviations: PIC-person in charge, WIC-walk-in cooler, RIC-reach-in cooler The PIC stated that they received a delivery of food late last night and all food is removed from the walk-in cooler as they put away the delivery. Ensure that the walk-in cooler is maintaining foods at or below 41F.
- Refuse - Maintaining Refuse Areas and Enclosures
Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.
- Pests - Controlling Pests*
Observation: Harborage conditions exist. Observed numerous small flying insects (fruit flies/drain flies) in the back of the kitchen.
Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Contact a professional pest control company to inspect and treat the facility for the flying insects.

Observation: The multi quat sanitizer being applied to food contact surfaces in the 3 compartment sink is too strong and does not meet the requirements of 40 CFR 180.940. The quat sanitizer concentration was 500ppm.
Correction: Utilize only multi quat sanitizer that meets the requirements of 40 CFR 180.940 when applying to food contact surfaces. The required concentration is between 150-400ppm.
