Backup sewage complaint in toilet facilities. See site copy for more details. Met with the new manager Richard O' Connor at 11:00 12/29/2015 to discuss the complaint. There was a sewage backup in the ladies' room. The plumber arrived at about 17:00 to correct the problem. Paper towels were improperly flushed down toilets and were snaked out of the floor drain by the plumber. The ladies' room should have been closed by the manager ( a mistake).
Inspection revealed that the cause of the problem was abated by the plumber. However, one of three toilets in the ladies' room was not operational and still needs to be fixed. The mens' room had no problems. Close this complaint . The complainant was notified of the results by email.

Needs improvements especially in the dishwashing area.

    Observation: Sugar dispensers improperly stored between uses.
    Correction: Store in-use utensils or dispensing utensils in the following manners: In the food with their handles above the top of the food and the container. Use scoops with handles.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) (repeated violation)
    Observation: Some PHF's at improper cold temperatures
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Use lids to cover foods.
  • Warewash Machine, Data Plate Operating Specifications
    Observation: The dishwasher is not operated IAW the manufactures' specification.
    Correction: Operate IAW the manufactures' specification.
  • Warewash Sinks and Drainboards, Self-Draining
    Observation: The dishwasher drain board is not self-draining.
    Correction: Equipment drain boards must be self-draining to prevent accumulation of standing water.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections at the dishwasher are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Ventilation - Mechanical Ventilation
    Observation: Ventilation is not sufficient to keep the dishwashing area free of excessive steam
    Correction: Provide mechanical ventilation that is sufficient enough to keep this area free of excessive steam and condensation. Have engineer check the fan on the roof for proper exhaust ventilation.

The Purpose of the visit was to follow up on a fire inspection conducted on 3 OCT.2015. All exposed foods were discarded and all affected equipment was cleaned as required. The Hobart dishwasher was also checked and found to be working satisfactorily: wash = 138 chemical = 100 ppm chlorine. It was also recommended that the pasta cooking equipment that was leaking be removed from the restaurant.

    Observation: The handwash station at the rear food prep area is being used to clean equipment and utensils in that said handwash sink observed being used to wash knife during inspection.

A complain (Case ID # 33830) was received on 3/31/14 stating that four out of seven people became ill after eating dinner at P.F. Chang's on Sunday evening 3/30/14. Symptoms included chills, vomiting and diarrhea within few hours of eating. Upon further investigation each individual had different food items such as Hot and sour soup, Sweet and sour pork, Asian tea, Pad Thai shrimp and Orange chicken. Establishment was aware of said complain. During inspection all potentially hazardous food tested at adequate temperature. Dishwasher was functioning adequately. No bare hand contact observed. Employees observed washing hands and changing gloves. Said complain is the only reported incident. No stool test was conducted. As per the employee record, no employee had called out sick on that week and after. Therefore, the complain was not found.
Operator reminded to train the employees on hand washing and glove use and on handling ready to eat food.
Complain was addressed.

    Observation: The cook is observed eating pizza in kitchen in front of the grill and low boy refrigerators with open top section holding food, where they may contaminate food, clean equipment, utensils or other items needing protection.
    Correction: Provide a designated area where employees may eat so as not to contaminate exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles, or other items needing protection.
  • Critical: Food - Safe and Unadulterated* (corrected on site)
    Observation: One bottle of Bacardi and one bottle of Chivas Regal observed contaminated with dead fruit flies in them
    Correction: Ensure food is safe and unadulterated.Operator voluntarily discarded said food item.

    Observation: Soiled or improperly cleaned equipment or utensils used during food preparation in that a dirty box was placed over water chestnut to drain its water.As per the operator said chestnut would then be grinded and added to various food.
    Correction: Protect food from cross contamination by using properly cleaned and sanitized equipment and utensils.

    Observation: 3030 Paper towel not provided in hand washing facility in kitchen.
    Correction: Operator provided paper towels in said hand washing facility.

    Observation: Soap was not provided at the hand washing lavatory in the bar area.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.Operator provided soap in said location.

AHi tuna notice indicates DO NOT THAW in vacuum packaging. Tuna in vacuum package at 33F at walkin.
Rice mesh run through dishwasher after each use.
Observed excellent glove use w/appropriate changes by cook line staff during lunch rush. Appropriate use of slash glove w/ single use gloves.
Noted newly installed spray sink at dishwash area-significant improvement noted in dishwash area drainage.
Excellent date labeling.

    Observation: Food handler (server with blue bandage) observed with bandage on finger and did not cover with glove or finger cot.
