    Observation: @POTENTIALLY HAZ FOOD@ noted not being adequately cooled to prevent the growth of harmful bacteria - Chili and hamburger (other food items in refrigeration were proper temperatures for cold holding).
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. The food items were disposed of at time of inspection.
  • Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition* (corrected on site)
    Observation: The prepared ready-to-eat (RTE) @FOOD@ in the refrigeration unit was not discarded by the ""consume by"" date.
    Correction: Discard the food at this time and ensure all prepared RTE food is served, sold or discarded by the ""consume by"" date. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria. The food was discarded at time of inspection.

    Observation: @PERSONNEL@ entering the food preparation, food storage, and/or warewashing areas are not in compliance with this Regulation. Maintenance personnel passing through the kitchen to parking area. Manager said that other personnel other than the kitchen workers are using the handwashing sink.
    Correction: Establish a barrier to food contamination by Informing employees, delivery and maintenance persons and pesticide applicators to comply with this Regulation when entering areas of the food establishment where food and food contact items are exposed. Individuals present in areas of a food establishment where food and food contact items are exposed presents a potential contamination risk.
  • Cooling, Heating, and Holding Capacities (repeated violation)
    Observation: @EQUIPMENT@ was overstocked and is not sufficient in number or capacity to meet the food storage demands of the establishment. The boxes of food are stacked to the doors of the walk-in freezer units.
    Correction: Provide additional @EQUIPMENT@ necessary to maintain food items at @TEMPERATURE@. Overstocked equipment does not allow for proper air circulation and/or heat transfer.
  • Critical: Equipment and Utensils - Hot Water and Chemical Sanitizing* (corrected on site)
    Observation: When tested, no sanitizer or a low concentration of chemical sanitizer was found in the wiping cloth bucket storing wiping cloths used on food contact surfaces..
    Correction: Provide @SANITIZING AGENTS@ at proper concentration of @SANI CONCENTRATIONS@ and immerse or expose food contact surfaces to sanitizing solution for adequate time.
  • Plumbing System Maintained in Good Repair (repeated violation)
    Observation: The faucet at the 2-compartment prep sink is constantly running.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Refuse - Covering Receptacles (corrected on site)
    Observation: Dumpster or outside refuse container was open or uncovered.
    Correction: Cover all waste containers when not in continuous use.
  • Refuse - Using Drain Plugs
    Observation: The refuse container used to store refuse/recyclables has no drain plug - 2 of the dumpsters had broken or missing plugs (1 was stuffed with a plastic bag).
    Correction: Replace the drain plug to the refuse container.
  • Refuse - Maintaining Refuse Areas and Enclosures
    Observation: Trash and litter were observed adjacent to the refuse container and area outside the facility. Personnel said that the public also uses the dumpsters.
    Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.
  • Outer Openings - Protected (repeated violation)
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents - a "sweep flap" was added to the base of the door from the inside but the metal is blocking the door from closing completely and there is a space between the pieces.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: The following physical structures are not maintained in good repair: cracks in wall/floor.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: The following physical structures were noted in need of cleaning: floors under the equipment and plumbing/conduits under the sink.

A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: A food employee failed to wash his or her hands before engaging in handling clean trays, after handling soiled utensils/plates or after engaging in any activity which may have contaminated his/her hands - dishwasher was not washing hands before putting on gloves to remove clean trays from baskets.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
