    Correction: Provide toilet room door that is tight fitting. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas. Adjust the closure to the employee's toilet room.

    Observation: The following physical structures are not maintained in good repair: shower house/restrooms - gap at the top of the floor wall juncture at the handicapped shower units in the men's and women's. A hole was cut out in the ceiling for a home-type fan to take out the heat in the storage room where the ice maker is located.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Properly ventilate and repair the ceiling.
