    Observation: There is a floor mounted cooking unit that is not elevated off the floor or sealed to the floor. A cleaning hazard exists.
    Correction: Alter the installation of the equipment to allow easy floor cleaning. Make 1 of the following corrections: 1. Remove the equipment 2. Install casters 3. Elevate the equipment to a minimum of 6 inches off the floor 4. Seal the equipment to the floor.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Unused or non-functioning equipment not removed from the premises - walk-in refrigeration is not working.
    Correction: Remove any unused or non-functioning equipment from the premises - if the unit is not going to be repaired or repairable remove the cooling unit and then repair surfaces to be smooth, nonabsorbent and cleanable. Lighting must meet the restaurant regulations.
  • Indoor Areas - Surface Characteristics
    Observation: The indoor floor material does not meet the standard of: 1. smooth, durable and easily cleanable 2. nonabsorbent - paper towels being used under a table-leg as a leveler.
    Correction: Replace the floor material at the area designated so it is: 1. smooth, durable and easily cleanable 2. nonabsorbent.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: The walls are not maintained in good repair in the dishwashing room - grout missing from the walls.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The walls were noted in need of cleaning in the dishwasher room especially behind the dishwasher - appears to be build-up of mildew.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: The dishwashing machine was observed in a state of disrepair and/or damaged - booster heater.
    Correction: Repair the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Critical: Backflow Prevention, Air Gap* (corrected on site)
    Observation: An air gap between the water supply outlet and the flood rim level at the prewash sink is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections under the dishwasher are leaking.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Physical Facilities in Good Repair
    Observation: The following physical structures are not maintained in good repair: grout in dishwasher room areas, condensate is leaking from the ceiling area (vents/light fixture area) in front of the 2 compartment sink, re-caulk junctures at the handsinks where needed, and walls at the mop sink are worn.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
