The purpose of today's visit was to conduct a risk factor assessment. Please continue the following practices as a means to incorporate Active Managerial Control:
1. Maintaining sanitizer solution logs (concentrations recorded twice daily)
2. Encouraging proper hand washing
3.Maintaining and monitoring food/equipment temperature logs.

The purpose of today's visit was to conduct a routine assessment. Please remember to take food temperatures of reheated items prior to placing in hot holding cabinet and/or on serving line. Commercially prepared items are to be reheated to 135F or above while items cooked from raw in-house are to be reheated to 165F or above. Also, please make sure to closely monitor items that are cooling (i.e., breakfast sandwiches) by taking temperatures to verify that the proper cooling process is being achieved.
  • Refuse Container with Cover/ Women's & Unisex Toilet Room
    Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
    Correction: PLEASE PROVIDE COVERED TRASH CAN FOR EMPLOYEE UNISEX RESTROOM WITHIN 30 DAYS. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulb(s) in the AMERIKOOL WALK-IN FREEZER are not covered by a protective shielding.
    Correction: PLEASE REPLACE MISSING LIGHT BULB SHIELD FOR LIGHT BULB LOCATED IN WALK-IN FREEZER WITHIN 30 DAYS. Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.

  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: McCALL 2 DR COOLER, BEHIND SERVING LINE
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.--EHS RECOMMENDS THAT THE THERMOMSTAT BE TURNED DOWN ON THIS UNIT SO THAT IT MAINTAINS PROPER TEMPERATURE DURING PEAK DEMAND. YOGURT IN THE UNIT WAS MOVED TO ANOTHER UNIT HOLDING AT 41F.
  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the THREE COMPARTMENT SINK with a concentration of 0 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.--CFM PLACED NEW BOTTLE OF SANITIZER ON SINK HOOK-UP. SINK WAS RE-FILLED AND READ 200PPM ON TEST STRIP.
