Observed kitchen well organized and clean. CFM is clearly knowledgeable in food safety, and ensures she and staff closely follow Regulations. Kitchen is "heat and serve" using only commercially packaged foods and pre-cooked animal products. Uneaten foods are discarded immediately. Please send evidence of repair or adjustment to the sanitizer dispenser to the EHS no later than 29 January 2016.
- Equipment / Good Repair / Operation
Observation: Observed that the sanitizer dispenser is not properly functioning. Dispensed solution was measured, and contained well over 500ppm of quaternary ammonium.
Correction: Repair/adjust the unit so it dispenses sanitizer with between 150 and 400 ppm of QA. Please send evidence of repair to EHS no later than 29 January 2016. CFM was aware of the malfunction and diluted the dispensed solution to an appropriate concentration, as verified with test strips.
- Plumbing / Maintained in Good Repair
Observation: Observed a leak from the faucet of the three vat sink while dispensing water.
Correction: Maintain plumbing in good repair.
- Physical Facilities Good Repair
Observation: Observed that the floor/wall junctures under the prep and three vat sinks are visibly soiled. CFM has made obvious effort to maintain floors clean, and indicated that these areas are no longer capable of being cleaned.
Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

- Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3-vat with a concentration of 100 ppm total quaternary ammonium compound.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm based on the manufacturer's use directions and using the appropriate test kit.
