The facility is in significant compliance with the VA food regulations. The kitchen is very clean and organized. The kitchen manager is knowledgeable about employee health and food safety policies.

01/19/2016Routine
Generally, facility appeared to be clean and well ordered. Person in charge knowledgeable about employee health policy, good food temperatures observed. Wax tape melted when tested dishwashing machine so appears to be sanitizing adequately. Bucket of sanitizer--quaternary ammonia at proper concentration of 200ppm. when tested in auto dispenser, ok at 300ppm.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: Unused or non-functioning equipment not removed from the premises-dishwashing machine on left side of kitchen not being used and gaps on sides not cleanable.
    Correction: Remove any unused or non-functioning equipment from the premises.

Generally, facility appeared to be clean and well ordered. Staff knowledgeable about employee health policy, good personal hygiene practices including handwashing and glove use were observed. Quaternary ammonia concentration at 3 compartment sink ok at 200ppm. Wax tape melted and turned black in automatic undercounter dishwashing machine so it appears unit is santiizing adequately.
  • Critical: Temperature* (corrected on site)
    Observation: Refrigerated grated cheese received at inadequate temperatures from commissary-not transporting in insulated transporting equipment or in refrigerated truck.
    Correction: Discard or reject delivery of potentially hazardous food received above 41°F. Routinely inspect PHF upon receipt for evidence of current or previous temperature abuse. Since transport had been only a short distance product was quickly chilled on site when received.
  • Critical: Food - Potentially Hazardous Food - Hot Holding* (corrected on site)
    Observation: Cooked broccoli hot holding at improper temperatures--just delivered.
    Correction: Rapidly reheat the vegetables to 135°F or above through the hot holding period.

Generally, facility observed to be clean and well ordered. Staff knowledgeable about employee health policy. Person in charge of kitchen monitoring internal food temperatures. Hot water sanitizing dishwashing machine appeared to operating properly-wax tape melted and turned black indicating full sanitization was acheived. Operator agreed to check milk temperature upon arrival and not to accept if internal temperature exceeds 41oF or above.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Upright single door refrigerators -design is such that condensate drips into pan continuously--operator to ensure condensate does not drip onto food or bottom of unit. Rust on front doors of unit. There is a risk of condensate contaminating food. When plan to replace unit, operator agreed to submit specification sheet to Health Dept. for prior approval before purchasing. Condensate should drain or evaporate from unit. If condensate pools on floor, unit may be freezing up and not holding food at proper temperature. This unit is not designed to be fully stocked -Unit is not able to cool milk when delivered at elevated temperature. It is a unit that is difficult to clean and maintain.
    Correction: Repair/replace the @EQUIPMENT@ to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the @EQUIPMENT@, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
