    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw shell eggs over ready to eat foods in 1 door work top refrigerator (bread area)
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc.
  • Equipment / Good Repair / Operation (corrected on site)
    Observation: Manager could not locate stoppers for 3 compartment sink upstairs.
    Correction: Provide stoppers for 3 compartment sink upstrairs for proper washing, rinsing, and sanitizing equipment and utensils (stoppers from prep sinks downstairs may be used for now)
  • Hand Drying Provision / Individual, Disposable Towels (corrected on site)
    Observation: Observed that paper towels were not provided at each handwashing sink (no paper at handsink in produce)
    Correction: Provide paper towels at each hand sink to allow employees to properly dry their hands after handwashing.
  • Critical: Storage of Toxics, Not Above Food, Linens, Equip., Single-Svc. (corrected on site)
    Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items (cleaning products stored above 3 compartment sink upstairs)
    Correction: All poisonous and toxic materials shall be stored below food, equipment, utensils, linens, and single service items to prevent contamination.

This visit was made to conduct a follow-up for a change of owner inspection. The following items shall be corrected within 30 days:
- Provide stoppers for 3 compartment sink upstairs.
- Replace missing and burnt out bulbs in bulk room and provide shielding for all bulbs.
- Replace missing baseboards throughout upstairs storage areas.
--- Approved for change of ownership. Recommend issuance of Health Department permit. This report shall serve as your permit to operate until your official permit is received. Any changes to floor plan, equipment or menu/ special processes shall be pre-approved by Alexandria Health Department.
No violation noted during this evaluation.		03/26/2015Pre-Opening
This visit was made to conduct a change of owenrship inspection. The following must be corrected before the permit can be issued:
- Provide a hot water heater with at least 22 kw (or a combination of heater)
- Provide a proper air gap at both 3 compartment sinks
- Repair holes in ceiling throughout second floor
The following must be corrected within 30 days:
- Repair floor in dairy walk in (must be non-absorbent, smooth and easily cleanable).
- Provide stoppers for all 3 basins of 3 compartment sinks.
- Repair all holes in walls upstairs (back storage area).
- Replace missing and burnt out bulbs
