- Certified Food Manager/Certificate Process (repeated violation)
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

Employee Health Policy in Place. Observed one employee wash hands properly during inspection. Please remember to fax or email a copy of the repair for the chemical sanitizer dispenser after the repair/adjustment to ensure that it is dispensing quaternary ammonium at 200ppm within 10 days. - Equipment / Good Repair / Operation
Observation: The chemical sanitizer dispenser plumbed to dispense quaternary ammonium is out of calibration and is not operating to dispense the allowable concentration of quaternary ammonium.
Correction: Repair/adjust the unit so it is able of dispensing 200ppm quaternary ammonium. Fax or email a copy of the invoice within 10 days. I have provided you with my contact information.
- Critical: Sanitizers, 21 CFR Criteria for Food Contact (corrected on site)
Observation: The concentration of the quaternary ammonium solution was measured at above 200 ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment per the manufacturers directions on the label of the sanitizer being used.
Correction: The manager manually added water to the dispensed premix from the dispenser plumbed at the sanitize basin of the three vat sink and after adjustment, the solution measured 200ppm. Maintain the concentration of quaternary ammonium solution at 200 ppm. Verify concentration using your test kit.

- Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
Correction: A complete employee health policy must have the following elements:
1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
- Critical: Handwashing / When to wash hands (corrected on site)
Observation: A food employee failed to wash his or her hands before engaging in food preparation, after handling a broom (sweeping the floor), an activity that could contaminate the hands.
Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.
- Food Storage / Exposed to Splash, Dust, etc.
Observation: Food stored in a location where it is subject to splash, dust or other contamination as follows: Drinks stored next to the handwashing sink.
Correction: Food shall be protected from contamination by storing the food where it is not exposed to splash, dust or other contamination. You may relocate the drinks or install a splash guard at the sink to protect the beverage from being contaminated.
- Food Storage / Prohibited Areas / Dressing Room (corrected on site)
Observation: Food storage Drinks stored under jackets/dresses at the employee cloth hanging area/wall.
Correction: Food/beveragre may not be stored in contact with personal clothing to prevent contamination.

- Handwashing Sink / Accessible at All Times (corrected on site)
Observation: The handwashing facility located in front of the walk-in refrigerator was blocked, preventing easy access by employees for easy handwashing. A rack of beverage bottles were noted stored in front of this handsink.
Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use.
- Handwashing Sink / Used for Hand Wash Only (corrected on site)
Observation: Observed the handsink at the hand washing sink located in front of the walk-in was being used to store utensils/wire scrub.
Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.
- Refuse / Cover Receptacles / Outdoors / Tight-Fitting Lids
Observation: Outside cardboard recycling dumpster refuse container was uncovered.
Correction: Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered with tight fitting lids or doors if kept outside the food establishment.
- Cleaning Frequency for Physical Facilities
Observation: Observed that the floor near the 3-vat sink is in need of cleaning. A soiled mop head was noted on the floor in this area.
Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.
- Critical: Cleaning Maintenance Tools, Preventing Contamination
Observation: Observed that two brooms were stored next to the mixing machine and hand sink in the back work area.
Correction: Clean and store maintenance tools in a separate area to prevent contamination of food and food preparation and warewashing areas.
