Observation: Floor or floor covering at threshold between kitchen and back room is not smooth and easily cleanable (tiles chipped and partially missing).
Correction: Obtain a transition strip for this part of the floor to make it smooth and easily cleanable.
- Outer Openings - Protected (repeated violation)
Observation: Outer opening of the food establishment is not protected against entry of insects and rodents (red screen door at back of facility needs to close more tightly ... gaps along side and at bottom when closed).
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

Observation: There is no covered refuse container in the employees' rest room.
Correction: Install a covered refuse container in the employees' rest room.
- Indoor Areas - Surface Characteristics
Observation: The indoor floor material in the back room and in the doorway between the back room and the kitchen does not meet the standard of: smooth, durable, non-absorbent, and easily cleanable. The painted cement floor has rough, uneven patches, and the transition from that floor to the tile in the kitchen is not smooth and clean (no threshold). Cove molding lacking in the back room at juncture of walls and floor.
Correction: Refinish or replace the floor material at the area designated so it is smooth, durable, non-absorbent, and easily cleanable. Install a threshold or other appropriate transition between the flooring in the back room and the flooring in the kitchen. Install cove molding in back room.
