The purpose of today's visit was to conduct a routine assessment. As previously mentioned in last inspection report, please obtain copy of certified food manager card and send to EHS within 5 days. Photocopy of card can be emailed or faxed. Also, please take care not use handwashing sink for tasks other than handwashing (i.e., storing trays, chemicals). Blocking the hand sink deters employees from proper hand washing, which directly impacts food safety and public health. Suggestions made to continually train long-term employees on employee health policy as information on food-borne illnesses and symptoms can be lost over time.
- Certified Food Manager/Certificate Process (repeated violation)
Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
Correction: PLEASE OBTAIN CFM CARD WITHIN 5 DAYS. Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.

Observation: Observed the handsink NEAR THE 3-VAT SINK being used TO STORE TRAYS, BOTTLES OF CHEMICALS.
Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. ITEMS REMOVED FROM HAND SINK.
- Refuse Container with Cover/ Women's & Unisex Toilet Room
Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall.
Correction: PLEASE OBTAIN COVERED TRASH CAN IN CUSTOMER LADIES ROOM PRIOR TO NEXT INSPECTION. A toilet room used by females shall be provided with a covered receptacle for sanitary napkins.

Observation: An air gap between the water supply outlet and the flood rim level AT THE ICE MACHINE is not observed. --OBSERVED ICE MACHINE WATER OUTLET PIPE TOUCHING THE FLOOR ABOUT 3 INCHES FROM THE FLOOR DRAIN. WATER HAS CAUSED MILDEW GROWTH ON FLOOR UNDER ICE MACHINE.
Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch.

Observation: Observed that hand soap was not provided at each handwashing sink.
Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.
