    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

    Observation: The beverage dispenser nozzles were heavily soiled
    Correction: Clean the surface at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: The nonfood contact surface of the following equipment has accumulations of grime and debris: digital thermometer exterior surface, metal storage shelves throughout (build-up), cookie container exterior, all refrigeration/freezer door handles and fronts of units, in crevices of refrigeration door gaskets, condiment storage bins at drive-thru, food built up in the gap between metal plate that is attached to the back of the walk-in cooler door, door handles and doors in facility, chair tops and chair legs in dining room.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Clean utensils in the basket hanging above the three compartment sink were being stored with soiled objects and dish wands used for cleaning dishes.
    Correction: Store in a clean, dry location where not exposed to splash, dust or other contamination at least 6 inches above the floor. Objects were removed and dishes are to be rewashed.

    Observation: Cups stored next to handsink at drive thru and observed set in a sticky wet substance.
    Correction: Store single service items away from the hand sink and in a clean dry location.

    Observation: The backflow or backsiphonage prevention device installed on the mopsink is broken. A threaded faucet exists and the system has a Y valve installed that is kept under pressure.
    Correction: Provide the approved backflow or backsiphonage device for the specific application and type of device to prevent contamination of the drinking water system. A backflow prevention device shall be installed directly on the threaded faucet.

    Observation: The refuse container used to store refuse/recyclables has no drain plug.
    Correction: Replace the drain plug to the refuse container. Call the waste management company to have a new drain plug installed.

    Observation: Light bulbs in the fixture behind the make line in the front of kitchen are not shielded, coated, or otherwise shatter-resistant.

    Observation: Soap was not provided at the hand washing lavatory in the front of kitchen at drive-thru. The soap dispenser is blocked and inaccessible. A bottle of soap is to be provided for handwashing if the soap dispenser is going to be continued to be blocked.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

Water was observed under the floor tiles in front of the walk-in cooler door and as you step on the tiles water pushes out from under tiles. Wall material worn on right side of kitchen by exits. Some wallpaper is peeling in dining area. Some floor tiles are damaged. Strips missing at drive thru window and gap between wallboards is exposed. Caulking split at restroor sinks.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

This inspection was a risk factor inspection that focused on the 5 risk factors for foodborne illness (food from unapproved sources, inadequate cooking temperatures, inadequate holding temperatures, improper employee health and hygiene, and cross contamination of food and equipment). Good retail practices (GRPS) observed were discussed with operator and/or staff at time of evaluation. Facility is current having issue with phorrid/fruit flies and being serviced by Ecolab. Observed most recent service report. Per PIC floor drains recently cleaned as recommended by Ecolab. Bulk of fly population observed at front counter near soda machine/hand sink and pick up window. Continue efforts to eliminate and or control population.
