EHS observed numerous drain flies throughout the facility to include on walls, counter tops, and on food contact surfaces. EHS discussed with the person in charge voluntary closure. The person in charge agreed to close the establishment until proper cleaning and treatment can be accomplished. EHS further explained that all exposed foods must be discarded. Contact EHS when facility has been treated and cleaning has been accomplished.

    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Correction: Separate raw foods during storage, preparation, holding, and display from raw RTE food including other raw food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: In-use utensils improperly stored between use. Observed dirty utensils stored on the counter in the serving area near the cash register.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: The following foods were cold holding at improper temperatures: 1-sliced tomatoes on the serving line (55'F), 2-ham in the display case (47'F), 3-turkey in the display case (47'F), 3-tuna salad in the display case (51'F). PIC stated that the food had been in the case overnight. Advised PIC that the items should not be used and recommended that they should be discarded. PIC took no action to correct the problem.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: The nonfood contact surface of the towel under the cutting board and the shipping plastic on the microwave oven are not corrosion resistant, nonabsorbent, and/or smooth.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
  • Temperature - Food Temperature Measuring Devices - Provided
    Observation: The Person in Charge could not produce an appropriate food temperature measuring device to verify cooking temperatures of thin foods such as meat patties or fish filets.
    Correction: A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: 1-the can opener blade, 2-the bulk container lids in the dry storage area, 3-the lids to the tea urns.
    Correction: Clean and sanitize these surfaces for food contact.

    Observation: The drain hoses located in the drip tray for the soda dispenser were observed soiled with accumulations of grime, debris, and what appeared to be mold.
    Correction: Clean the surface of the drain hoses at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: The cavity of the microwave oven is observed soiled.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.

    Observation: The following nonfood contact surfaces have accumulations of grime and debris: 1-inside of the cabinet under the flattop grill, 2-the cover on the bread cart, 3-all of the shelves in the walk-in cooler, 4-all of the shelves throughout the kitchen and serving line, 5-the tea urn drip trays, 6-inside of all of the cabinets on the front line, 7-the vents on the soda dispenser, 8-the condenser fan covers in the WIC, 9-the drawer holding the sauce squeeze bottles on the front line
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Several metal and plastic pans were found stacked while wet after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.

    Observation: The handwashing facility located at the front line is blocked, preventing access by employees for easy handwashing. Observed a chemical spray bottle stored in the handsink.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the chemical spray bottle preventing its use.

    Observation: Insect control device is located over the 3-compartment where dead insects may be impelled or fall.
    Correction: Install the insect control device away from a food preparation area so that dead insects and insect fragments are prevented from being impelled onto or falling on exposed food, clean equipment, utensils, and linens, and unwrapped single service and single use articles. Exposed food and food-contact surfaces must be protected from contamination by insects or insect parts. Installation of the device over food preparation areas or in close proximity to exposed food and/or food contact surfaces could allow dead insects and/or insect parts to be impelled by the electric charge, fall, or be blown from the device onto food or food-contact surfaces.
  • Handwashing Cleanser - Availability
    Observation: Soap was not provided at the hand washing station on the front line.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

    Observation: No disposable towels were provided at the hand washing lavatory in the men's restroom.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: The following physical structures are not maintained in good repair: 1-the coving tiles throughout the front line and the backroom, 2-there are water stains on some ceiling tiles in the back room. When asked about it, the PIC stated that the ceiling leaks when it rains.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The floor sink under the prep sink was noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
  • Critical: Toxics - Common Name/working Containers of Toxics*
    Observation: Working spray bottles of chemicals are not properly labeled. Observed chemical spray bottles on the front line and in the back room that were not labeled.
    Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
