Bordeaux and Normandy kitchens are serving kitchens only. All food is prepared at Commonwealth Health and Rehab main kitchen. All dishwashing is done at Commonwealth main kitchen, 3Vat sinks and dish machines in Bordeaux and Normandy kitchens are not in use.
ServSafe certificate observed for CFM. Please send copy of Certified Food Manager's card to Health Department by 12/1/14. Observed current CFM cards for additional employees.

  • Wiping Cloths/Use Limitation/Wet Towel Storage
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. PLEASE CREATE SANITIZER BUCKETS FOR WIPING CLOTHS PER KITCHEN.

All dishwashing is done at the Commonwealth Health & Rehab kitchen. No dishes are washed at Bordeaux or Normandy kitchens. Dish machines located in each kitchen is not in use.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Certified Food Manager/Presence Required
    Observation: There is no Certified Food Manager present in the establishment. CFM CARD IS EXPIRED, CURRENT CARD NOT AVAILABLE.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. PLEASE FAX OR EMAIL COPY OF CURRENT CFM CARD TO HEALTH DEPARTMENT.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees. Observed the handwashing sign inside the cabinets at the Bordeaux dining area.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. CFM PROVIDED HANDSIGN AT KITCHEN HANDSINK.
02/05/2013Routine
The purpose of this visit was to conduct a routine inspection in conjunction with a complaint. The complaint inspection was generated through the CD/Epi department due to a confirmed Norovirus outbreak on one side of the facility. The manager is aware of the situation and complaint and has discussed the outbreak with the health department. The manager has taken all precautionary methods to prevent the spread of Norovirus by resorting to single service disposable dishware upon the request of the Health department until the ban was lifted today 2/5/13. The establishment receives all their meals catered from the Commonwealth Health care kitchen. Prior to entering the kitchen a sign was observed on the door restating the employee health policy, indicating what symptoms and infections an employee must report to a manager. During the inspection it was noted that employees wash their hands, wear gloves and hairnets when handling food. One out of the two dish machines is in use. The dish machine in the Bordeaux dining room is occasionally used to wash, rinse, dishware. The dishware is washed and rinsed in the 3 compartment sink at both the Normandy and Bordeaux dining area on site then returned to the catering company for another wash, rinse, sanitize cycle. Upon discussion with the employees it was determined that they have knowledge and an understanding of the employee health policy.

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees. OBSERVED THE HANDWASHING SIGN INSIDE THE CABINET ABOVE THE HANDSINK, NOT VISIBLE TO THE STAFF AT THE BORDEAUX DINING AREA.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. CFM REMOVED HANDWASHING SIGN FROM CABINETS AND PLACED ON THE WALL ABOVE HANDSINK TO MAKE IT VISIBLE TO THE PUBLIC.
