A site visit was made to conduct a routine risk factors inspection. At the start of the inspection EHS observed multiple foods being thawed improperly at the prep sink. After taking the temperature of the tap water and determining that it was too warm for proper thawing (max. 70 °F) at the sink with running water ice would need to be added. (As) Facility staff began to drain from the sink water backed up from the floor drain onto the floor. Due to this sink being direct plumbed EHS instructed PIC to discard the food and sanitize the sink. The Plans Ops staff have an commercial snake and are working to locate the issue in the pipes beneath the kitchen. This back-up occurred at the end of lunch service. PIC has halted all production until the Plant Ops staff have completed their work and has changed the dinner menu to a heat and serve product to allow for more time to clean the kitchen. Plant Ops staff are currently working their way around the kitchen attempting to locate the clog/blockage. PIC understands that all food contact surfaces in the area need to be washed, rinsed and sanitized. EHS also advised PIC to sanitize/bleach other surfaces as needed (ex. shelving, equipment handles, floor). Contact EHS with update/progress report within 24 hours. ACR 33461 A complaint was received that stated that a dog has been present in the dining room of this facility on multiple weekends, The complaint further stated that the dog was permitted to be on dining room tables and eat off resident plates. EHS made a site visit and spoke with the Dietary Manager/PIC. The PIC stated that she was not at the facility yesterday but that an employee has reported the presence of a dog during dinner. According to the PIC yesterday is the first instance of a dog being brought into the dining room during her work experience at this facility (about 18 years) and that according to the employee report the dog was not on the table and did not consumer facility food. PIC indicated that the facility management is aware of the situation and letters will be sent to the family of the resident that brought the dog into the facility. PIC further indicated that an in-service training session will be conducted the afternoon with both shifts present regarding procedures to follow when a dog is brought into the dining room (food service worker contacts nurse supervisor and nurse supervisor ask them to remove the dog. ***Please be aware of ADA/support animals vs. pet animals.*** No violation noted during this evaluation. | 03/10/2014 | Complaint | |

The ceiling tiles in these areas appear to be perforated ACT - please install washable tiles in these areas as discussed. A new handsink has been installed in the service area in order to use this station for light food preparation (waffles) - please provide a paper towel dispenser at this hand sink.
