Observation: The person in charge failed to obtain a valid certified food managers card. The person in charge failed to employ a certified food service manager.
Correction: Obtain a valid food service managers card. Note: Has enrolled in Jan 5th/7th 2016 CFM Class,while on site via Norfolk Public Health Dept. CFM Class/phone conversation with office staff. (Front Office Manager/EH).

Observation: Employees observed working in the food service area without proper hair restraints.
Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
- Temperature Measuring Devices; Ambient Air and Water
Observation: No ambient (air/water) temperature measuring device (dual or degrees Fl) located in the Two (2) Standup Reachin True refrigerators in Back Area/Room of Kitchen
Correction: Provide a temperature measuring device inside of refrigerators that are easily readable and accurate to +/-3 degrees Fahrenheit in the intended range of use.Corrected on site.
- Floor and Wall Junctures, Coved, and Enclosed or Sealed
Observation: Floor Tiles and wall Tiles- juncture in Kitchen area, next to and under handsink in front area of kitchen, also tile covening in back area entry way to diswasher room is not sealed.
Correction: Seal floor and wall juncture in rooms where water flush cleaning methods are used.

Observation: Less than 20 foot candles of light was noted in the ladies and mens restroom.
Correction: Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms

Observation: Less than 50 foot candles of light was noted in the 2 dr lowboy sandwich cooler for preparing/cutting meat/Salad sandwiches
Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor
- Physical Facilities in Good Repair
Observation: Wall Tiles,Covening Tiles, and Floor Material in kitchen which was repaired previously,is not smooth and easily cleanable. Is not maintained in good repair
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Premises - Maintaining Premises; Unnecessary Items and Litter
Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment.(Cardboard Boxes/Material stored in front of Electrical Panel).
Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents/Fire Hazard.
- Mops - Drying Mops
Observation: Mops and brooms not hung up to air dry.(Sitting on floor wet).
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
