Natalie Olsen- Manager on site.(No CFM Certificate). Has signed up for Jan. 2016 CFM Class and Paid. Head Chef/ CFM Andrew Wall certificate Expires 2/12/2018 (Norfolk). Post Current CFM certificate and Health Dept Permit for Public View.Note: Maintain temperature LOgs/Charts for all refrigeration units daily (Closing and opening of facility and change of shifts ).
  • Critical: Demonstration of Knowledge* (repeated violation)
    Observation: The person in charge failed to obtain a valid certified food managers card. The person in charge failed to employ a certified food service manager.
    Correction: Obtain a valid food service managers card. Note: Has enrolled in Jan 5th/7th 2016 CFM Class,while on site via Norfolk Public Health Dept. CFM Class/phone conversation with office staff. (Front Office Manager/EH).

    Observation: Employees observed working in the food service area without proper hair restraints.
    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food clean equipment, utensils, and linens, and unwrapped single-service and single-use articles.
  • Temperature Measuring Devices; Ambient Air and Water
    Observation: No ambient (air/water) temperature measuring device (dual or degrees Fl) located in the Two (2) Standup Reachin True refrigerators in Back Area/Room of Kitchen
    Correction: Provide a temperature measuring device inside of refrigerators that are easily readable and accurate to +/-3 degrees Fahrenheit in the intended range of use.Corrected on site.
  • Floor and Wall Junctures, Coved, and Enclosed or Sealed
    Observation: Floor Tiles and wall Tiles- juncture in Kitchen area, next to and under handsink in front area of kitchen, also tile covening in back area entry way to diswasher room is not sealed.
    Correction: Seal floor and wall juncture in rooms where water flush cleaning methods are used.
  • Lighting, Intensity
    Observation: Less than 20 foot candles of light was noted in the ladies and mens restroom.
    Correction: Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms

    Observation: Less than 50 foot candles of light was noted in the 2 dr lowboy sandwich cooler for preparing/cutting meat/Salad sandwiches
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor

    Observation: Wall Tiles,Covening Tiles, and Floor Material in kitchen which was repaired previously,is not smooth and easily cleanable. Is not maintained in good repair
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Premises - Maintaining Premises; Unnecessary Items and Litter
    Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment.(Cardboard Boxes/Material stored in front of Electrical Panel).
    Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents/Fire Hazard.
  • Mops - Drying Mops
    Observation: Mops and brooms not hung up to air dry.(Sitting on floor wet).
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

Viewed food handler cards. Cards viewed are current. Copy of online food handler information given to manager for new employees who need to take course. Manager to sign up for next food manager class. Copy of food manager class schedule given to manager. Refrigeration and product are holding at proper temps during inspection. Food permit to be issued once manager has signed up for class.

  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located inside True 2 door fridge next to hand sink.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

    Observation: Ice machine filter in need of cleaning (dusty).
    Correction: Clean and sanitize ice machine filter.

Copy of 2014 Food Manager's and Food Handler Class schedule given to manager. Certified food managers to sign up for manager's class. Will follow back up with manager in reference to signing up for class and becoming certified.

    Correction: Obtain a valid food service managers card.

Application left on site with manager to have filled out and sent in with permit fee of $40. Copy of food manger class schedule given to manager. Viewed food handler cards. Manager has to get signed up for food manager certification class. Facility recommended for permit. Permit will be issued once updated application and permit fee is received and processed.

    Observation: There is no covered refuse container for the disposal of feminine napkins in the ladies room stall. No lid on trash container.
