This is a follow-up inspection to 1st routine inspection performed on May 18, 2015. Observed compliance for: -Virginia Clean Air Act - membership file for exempt smoking facility status. -Employee Health Policy -airgap for the 3-vat -shelving *Please send me invoice for the box showing light bulbs for the kitchen are shatter-proof by June 14, 2015.

Observation: Facility does not meet requirements of a private club. This facility is members only smoking facility. Facility was not keeping record of the private membership.
Correction: Owner must comply with requirements of Title 15.2-2825.
- Critical: Responsibility of Employee to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
Observation: Employee Health Policy was not in place.
Correction: EHS provided training and documentation.
- Food Storage / Preventing Contamination
Observation: Food equipments and hookah smokes were on the ground
Correction: Food and food equipment shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
- Sanitizer Test Kit Required
Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a BLEACH test kit.
- Critical: Air Gap Design Standard, Not Less than 1'
Observation: An air gap between the water supply outlet and the flood rim level 3-vat sanitizing vat drain pipe is not observed.
Correction: An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch.
- Light Bulbs / Locations where Shielding is Required
Observation: Light bulb(s) in the kitchen are not covered by a protective shielding.
Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.

This visit was to conduct a pre-opening inspection. Please fix and email me pictures of correction by April 2, 2015: -Please have all lights in the kitchen working with light shields. -Please provide air gap for sanitizer vat piping. -Please send me a picture of dry storage shelving. Approval for Health Department permit is hereby granted. This inspection report shall serve as your permit until your regular permit is delivered to you.
