Rusted galvanized sinks must be replaced. Raw chicken is to be prepared at the Fork in the Alley location as stipulated on the permit for this establishment.
  • Critical: Demonstration of Knowledge*
    Observation: The person in charge failed to explain the significance of the relationship between the holding time and temperature of potentially hazardous food and foodborne illness. The person in the kitchen said he reheated food "till it burned him" and was unable to give the proper temperature for reheating or produce a thermometer. The facility agreed not to cook, or thaw raw chicken at this location. There is raw frozen chicken in the freezer that the PIC stated is thawed in the refrigerator and cooked in small batches.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.

    Observation: The nonfood contact surface of the galvanized sinks are rusted.
    Correction: REPLACE the galvanized sinks. Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
  • Temperature Measuring Devices
    Observation: There was no properly working temperature measuring device located in the bar prep unit.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Temperature - Food Temperature Measuring Devices - Provided (corrected on site)
    Observation: The person in charge could not provide a food temperature measuring device.
    Correction: Obtain at least one food temperature measuring device so employees can properly monitor the temperature of food items in the establishment.
  • Sanitizing Solutions, Testing Devices
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonia test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
  • Critical: Equipment and Utensils - Hot Water and Chemical Sanitizing* (repeated violation)
    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (See 12 VAC 5-421-1700)
    Correction: Discontinue use of the dish machine until it is repaired to properly sanitize dishes. Repair the unit to restore chemical sanitization. Verify the level of sanitizer and continually monitor by the use of test strips/kit daily.
  • Equipment and Utensils, Air-Drying Required (repeated violation)
    Observation: @TABLEWARE@ were observed cloth-dried rather than air-dried after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored. Cloth-drying may transfer microorganisms to equipment or utensils. Add shelving to accomodate drying dishes.
  • Equipment, Utensils, Linens, and Single-Service and Single-Use Articles (repeated violation)
    Observation: Single service items observed unprotected from contamination on the floor.
    Correction: Store single service items at least 6 inches above the floor.
  • Hand Drying Provision
    Observation: No disposable towels were provided at the hand washing lavatory in the bar area.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
