Notes: When setting up pizza table in the morning, turn unit on and wait until unit gets down to 41 F or less before placing food inside. Provide thermometer to monitor internal temperature. Critical violation must be corrected within 10 days, non-critical violations must be corrected within 30 days.
Temperature Measuring Devices Observation: There was no temperature measuring device located in the pizza make table.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
Critical: Backflow Prevention Device, When Required* Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin (mop sink)
Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
Floor and Wall Junctures, Coved, and Enclosed or Sealed Observation: Floor wall juncture under 3 VAT sink and near mop sink is not coved and closed to no larger than 1/32 inch space.
Correction: Cove floor wall juncture to no larger than a 1/32 inch space.
Handwashing Signage/Handwashing Facilities (repeated violation) Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees
Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.
Lighting, Intensity Observation: Less than 10 foot candles of light was noted in the walk-in cooler.
Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.

Correction: Plumbing systems and components shall be maintained in good repair.
Critical: Sanitizer - Criteria/Chemicals for food contact* Observation: QUATERNARY AMMONIUM sanitizer being applied to food contact surfaces does not meet the requirements of 40 CFR 180.940. Sanitizer tested > 400 ppm.
Correction: Utilize only 200 ppm that meet the requirements of 40 CFR 180.940 when applying to food contact surfaces

Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the soiled linen receptacle preventing its use.

Correction: Working containers of toxic items are to be properly labeled with contents. Identification of these containers with the common name of the material helps prevent the dangerous misuse of the contents.
