Correction: Provide a temperature measuring device that is accurate within +/- 2 degrees Fahrenheit in the intended range of use.
Water Pressure (repeated violation) Observation: Hot / Cold water under insufficient or inadequate pressure was found to serve the following equipment: handsink in employee bathroom.
Correction: Inadequate water pressure could result in improper handwashing or equipment operation. Restore water under sufficient and adequate pressure to the handsink immediately.

Reviewed menu. All proteins received precooked frozen. Discussed reheating to 165 degrees and spot checking temperatures.Supplier is Sam's Club. Discussed how to calibrate bi metallic thermometer. Recommend checking hot water heater for temperature as when lots of water in use, temperature not sufficient at 3 vat sink. (need 110 at all times) Minimal leftovers. Discussed cooling methods. No family style service. Discussed no bare hand contact. Good use of handwashing and glove use. Employee Health Form 1B observed and in use. Facility waterworks regulated by ODW.

Food suppliers are Sam's Club and PFG. Proteins are received pre-cooked and most are frozen. Facility serves AM & PM snack and lunch. Food is cooked/ reheated and served No violation noted during this evaluation.

Employee Health Information was present and posted. Sanitizer ~ 100 ppm.* Reminder: Thermometers are needed for Reach-In Cooler and Chest Freezer. Ensure all Employees involved in preparation of foods to wear a hair restraint. Discussed proper cooling and cold holding temperatures for foods. No bare contact of foods. Discussed with PIC that they have good date marking.
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) (repeated violation) Observation: Observed pasta with tomato sauce and green beans cold holding at improper temperatures. Person in Charge was unaware of how long and why food was out of temperature
Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
Handwashing - Using a Handwashing Lavatory - No Other Purpose (corrected on site) Observation: Observed handsink being used for other purposes other than handwashing. Observed paints and other painting items in the handwashing sink.
Correction: Recommend using handsink only for the purpose of handwashing only.

No food prep observed during the inspection. Reviewed menu: lunch menu for the day consisted of pasta, green beans and canned pineapple
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Observed milk cold holding at improper temperatures
Correction: observed milk at 45.6 F. Person in charge stated it was out/ used during lunch but was not aware how long it had been cold holding above 41 F.
Warewashing - Manual Warewashing Equipment, Wash Solution Temperature Observation: Observed the temperature of the in-use wash water solution was observed at less than 110°F.
Correction: Maintain a temperature of 110°F and above or the temperature specified by the manufacturer for optimal performance of the detergent to clean equipment and utensil surfaces. Recommend adjusting water heater to maintain a temperature of 110 F at the wash basin of the 3 basin sink.
