- Critical: Hands - Preventing Contamination from Hands* (corrected on site)
Observation: An employee was observed contacting a sub roll with bare hands.
Correction: We do not allow bare hand contact with ready-to-eat foods. Use disposable gloves, tongs, and spoons. This violation was immediately corrected by the establishment management.
- Nonfood Contact Surfaces
Observation: There was an inverted plastic bread crate being used as a substitute for smooth, easily cleanable shelving, in the walk-in refrigerator.
Correction: We do not allow plastic bread crates (or soft drink bottle crates, or milk crates) to be used as a substitute for smooth, easily cleanable shelving. These crates have rough surfaces. They are not cleanable at the bacterial level. They introduce an unnecessary place where bacteria may hide, and may grow, in the walk-in refrigerator. Replace with proper shelving, or smooth easily cleanable, moisture resistant materials. If wood is used, it must be painted, or sealed.

Observation: There was a carton of shell eggs being stored above pizza dough pans, in the walk-in refrigerator.
Correction: Shell eggs (and raw meats) must always be stored below, and well separated from all food contact surfaces, (and all ready-to eat foods). This violation was immediately corrected by the establishment management.
- Equipment - Good Repair and Proper Adjustment
Observation: There are torn gaskets inside the tops of both of the two chest freezers, exposing surfaces that are no longer smooth and no longer easily cleanable.
Correction: These gaskets need to be replaced with unbroken, smooth, easily cleanable gaskets.
- Critical: Single-Service and Single-Use Articles, Required Use*
Observation: There were black olive slices being stored in a white plastic bucket, which had a lid on it with commercial writing impregnated in the lid saying "cheese.
Correction: Only the food which originally came in these white plastic buckets, and lids, may be stored in these containers, and using these lids. These buckets (and lids) are intended to be for a single use only by the manufacturer, and by the food supplier. Switch to tupperware or some other more durable and easily cleanable type of container.
- Critical: Toxics - Identifying Toxic Containers*
Observation: There was a spray bottle containing a yellow liquid, with no label on it to identify the contents of the spray bottle.
Correction: We require that all containers of chemicals, soaps and, sanitizers be labeled to clearly identify the contents. This violation was immediately corrected by the establishment management.

- Hair Restraints - Effectiveness (repeated violation)
Observation: Employees, working in the kitchen areas, are not wearing the hair coverings that we require.
Correction: We require that all employees working in the kitchen wear a hats, a hair net, an open visor, a scarf, or some other type of hair covering that we have approved.
- Equipment and Utensils - Durability (corrected on site)
Observation: There was a sheet of cardboard, below the bulk bags of flour, and above the steel storage rack, that the bags of flour are stored on. Also, in the front part of the kitchen, there was a paper towel "liner" in the bottom of the storage box, where the steel forks are stored.
Correction: Paper towels, or cardboard, (or foil) may not be used as 'liners," in food establishments. Only materials that are resistant to moisture, and are durable enough the stand up to repeated washing, may be used for this purpose. A sheet of smooth plastic (or steel) would be ideal, if a "liner" is really needed below the bulk bags of flour, and above the steel mesh storage rack. No liner at all would be best in the small plastic storage box used for the steel forks. This violation was immediately corrected by the establishment management
- Kitchenware and Tableware (corrected on site)
Observation: The plastic forks (not individually wrapped), in the front part of the kitchen, near the "pick-up only" counter, had their handles facing in both orientations, in the storage box.
Correction: All of the handles need to be facing in the same direction. Otherwise, by accident, one can touch the eating surface of several forks, when reaching for the handle of a fork. This violation was immediately corrected by the establishment management. Note that we recommend, but we do not require, that disposable plastic utensils be individually wrapped.
