    Observation: The main door of the Leer walk-in refrigerator and door of the Metro C5 warming unit were observed in a state of disrepair and damaged.
    Correction: Repair the Leer walk-in box and Metro C5 warming unit doors to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the Leer walk-in box and Metro C5 warming unit doors, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The door gasket to the Leer walk-in refrigerator is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Water from the handwashing sink at front food prep area was measured at a temperature less than 100°F.
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.

    Observation: Observed a hose attached to a faucet fixture. The hose extended below the flood rim level of the sink basin.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.

    Correction: Obtain a quaternary ammonia test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: Metro C-5 warmer was observed in a state of disrepair and damaged in that the top door of the unit has one of two hinges that is broken and the door is hanging down.
    Correction: Repair the walk-in box and Metro C-5 warmer to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the walk-in box and Metro C-5 warmer, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: A direct connection exists between the drain line of the ice machine entering the utility sink in rear food prep area, the drain line of the walk-in refrigerator and the floor drain inside the walk-in box and the soft drink dispensers and the floor drains under the front service counter and the sewer system.
    Correction: Eliminate the direct connection between the sewer line and the cited equipment.

    Observation: Tile floor and tile cove base is not maintained in good repair in that many tiles are crack or dislodged from and cove base in the rear food prep area.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Numerous flies were noted in the establishment
    Correction: Prohibit all live animals on the premises of the food establishment. Animals carry disease-causing organisms and can transmit pathogens to humans through direct and/or indirect contamination of food and food-contact surfaces. Animals shed hair continuously and may deposit liquid or fecal waste, creating the need for vigilance and more frequent and rigorous cleaning effort.

This follow-up inspection was conducted to verify that the Leer Walk-in refrigerator was repaired and fully operational as reported by the PIC. Establishment replaced the compressor on the Leer Walk-in refrigerator on 15-July-2015. Walk-in box was observed holding at 35
°F
at the time of inspection, The operator is cleared to begin storing food in the Leer walk-in refrigerator.

  • Critical: Food - Potentially Hazardous Food - Cold Holding* (repeated violation)
    Observation: Assorted raw chicken pieces cold holding at improper temperatures in that twenty six 44 lb complete boxes and approximately one opened 44 lb box of raw assorted chicken pieces was measured at 49°F in the Leer Walk-in refrigerator at the time of inspection. PIC voluntarily denatured chicken in preparation for discarding.
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: Leer walk-in refrigerator was observed in a state of disrepair and damaged in that the unit was observed at an ambient temperature of 49°F.
    Correction: Repair the Leer walk-in to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the Leer walk-in refrigerator, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Sewer lines in poor repair in that the aroma of sewer gas is present at the floor drains under the front counter, Leer walk-in refrigerator and at slop sink in rear food prep area.

    Observation: The raw assorted chicken pieces in the Leer walk-in refrigerator and the fried chicken that was prepared from the Leer walk-n refrigerator is unsound or adulterated in that said chicken was stored in a faulty walk-in refrigerator at temperatures above 41°F for over 16 hours at the time of inspection. PIC voluntarily discarded the assorted fried chicken pieces and denatured with bleach cleaner twenty-seven (15 mixed, 12 dark) 44 lb cases of raw chicken and approximately one case of assorted chicken pieces which had been seasoned for frying in preparation for discarding.
    Correction: Ensure food is safe and unadulterated.
