The purpose of this visit was to conduct a complaint investigation in conjunction with a routine inspection. Discussed the Active Managerial Control (AMC) with the PIC.

The purpose of this visit is to conduct a risk factor assessment. Thank you for following me through the inspection as it allows me to answer any questions or clarify any questions that arise. During today's visit observed several employees washing hands using proper technique. Sanitizer observed within an acceptable limit. Manager knowledgeable on the minimum cook tempreature of chicken. Observed Active Managerial Control demonstrated by the certified food manager today. Employee Health Policy in Place. The manager is using time as a public health control for hot items in the hot drawers, hot holding wells appropruately. Opened, potetially hazardous food items were properly date marked. Minimum cook temperatures observed for chicken.
EHS provided a handout and an explanation of the Active Managerial Control Program, as well as a discussion. Thank you for your time. If you have any questions, please call the health department at 703-246-2444.

  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703) 385-9568. Failure to provide a copy of the photo identification card will result in further enforcement action.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Wiping Cloths/Use Limitation/Wet Towel Storage (corrected on site)
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counter.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the refrigerators located on the front serving line are torn/ damaged.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Replace the broken rubber gaskets/seals.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris:Top of the convectional oven located under the hood was observed to be greasy .
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Clean the surfaces of cooking equipment more frequently to prevent grease and debris accumulations..
  • Permit to Operate Required, Must be Posted, Must be Valid (corrected on site)
    Observation: At the time of the inspection, it was observed that a change of ownership has taken place at the food establishment. The current operator does not have a valid permit to operate.
    Correction: Within 48 hours, the following must be done to ensure continued operation of the establishment: 1) submit an Application for Permit to Operate a Food Service Establishment and a copy of a current business license to the Health Department
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There was no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulb(s) in the dry storage area are not covered by a protective shielding. Two of the ceiling light fixtures have missing shields.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles. Replace the missing light shield to protect food from contamination.

This visit was conducted to do a Pre opening inspection.
Approval is hereby granted for the issuance of a Health Department permit. This inspection report shall serve as your permit until the official permit is obtained. A routine inspection will be conducted in approximately 30 days. No equipment additions/changes/replacement are allowed without Health Department approval.
