Recommend a large ice scoop holster on the wall adjacent to the ice maker. Recommend a wall brace on the back of the dishwasher rinse-arm. Discussed critical items, Raw animal & RTE food storage, handwashing, glove use, & EHP handouts. Dishwasher appears to be working properly.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Different types of raw animal foods stored in such a manner that may cause cross contamination.
    Correction: Separate types of raw animal foods from each other such as beef, fish, lamb, pork, shell eggs,and poultry during storage by the following: (a) Using separate shelves and/or arranging each type of foods that cross contamination is prevented.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site)
    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the walk-in cooler.
    Correction: Separate raw foods of animal origin during storage from raw & cooked RTE foods including vegetables and fruit.

    Observation: Unlabeled bulk food containers.
    Correction: Label containers of foods that cannot be readily identified - flour, salt, sugar, oils, etc.
  • Food Storage - Clean and Dry Location
    Observation: Inadequate storage in walk-in units, 2-door upright cooler, & in dry storage.
    Correction: Provide adequate approved shelving, dunnage racks, and/or carts for food items.
    Recommend: 1) Additional shelves on existing shelving units in the walk-in cooler and the dry storage shelves by office.
    2) A shallow shelving unit at the end of the cooking line behind the WIC door..
    3) Carts for the walk-in cooler by the office & next to the dry storage shelving units.
    4) An over-shelf on the back of the 2-door work-top freezer.
    5) Additional shelves in the 2-door upright cooler.
  • Food Storage - Clean and Dry Location
    Observation: Food stored on the floor or less than 6" above the floor in the walk-in coolers;Store food at least 6" off the floor on approved shelving, dunnage racks, and/or carts.
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the hamburger that may be served undercooked
    Correction: If an animal food such as beef, eggs, fish, lamb, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, menu advisories, or other effective written means.

    Observation: Paper liners on bar glass cart shelves.
    Correction: Do not use paper or linen liners on shelves. Recommend bar mesh or approved racks.
  • Drainboards
    Observation: Dishwasher & 3-vat sink drainboards are not large enough to accommodate all soiled items.
    Correction: Provide storage so that all soiled utensils and equipment can be properly stored as they accumulate during operation. Recommend a dishwasher rack over the dirty dishwasher drainboard for glasses, cups, etc., and a shelving unit or cart for dirty dish pans below the 3-vat sink drainboard.
  • Responsibilities of Owner or Proprietor
    Observation: Absence of non-smoking international no-smoking signs in non-smoking area.
    Correction: Post of no-smoking or international no-smoking signs the dining area and/or at the patron entrances.
  • Equipment - Fixed Equipment, Elevation or Sealing
    Observation: The bar Coke cooler id not elevated off the floor. A cleaning hazard exists.
    Correction: Install castors or 6" approved leg on unit. Recommend a moving dolly.
  • Handwashing Cleanser - Availability
    Observation: Commercial soap dispensers empty in kitchen.
    Correction: Refill soap dispensers.
  • Hand Drying Provision
    Observation: Commercial paper towel dispensers empty / missing in kitchen & bar.
    Correction: Install / refill paper towel dispensers in kitchen & bar.
  • Lighting, Intensity
    Observation: Less than 20 foot candles of light in the rest-rooms. Measured 16-18 foot-candles.
    Correction: Provide at least 20 foot candles of light in toilet rooms
  • Critical: Toxics - Common Name/working Containers of Toxics*
    Observation: Pump-spray containers of cleaning products not properly labeled.
    Correction: Label containers of toxic items with the common name of the contents.

The FDA Form B1 Conditional Employee or Food Employee Reporting Agreement was explained and provided to the operator for the development and implementation of the Employee Health Policy.

No violation noted during this evaluation.

NOT APPROVED FOR FOOD PERMIT
1. This food establishment installed a smaller walk in cooler in the food preparation area instead of a upright freezer.
2. All cold holding units should be working properly in order to maintain temperature control for safety food (TCS) food at 41°F or below.
3. The dishwasher machine should be installed properly, detergents and sanitizer shall be installed and ready to be inspected. Install a detergent/sanitizer level indicator and data plate
4.Install approved shelving over the prep sink for dry food storage.
5. Install paper/dispenser hand towel at all handwashing sink stations.
6.Install a splashguards between the food prep sink and the handwashing sink.
7. All raw wood at the bar area should be replace with a smooth, durable and easy to clean material.
8. Install self closing devices at the toilet room doors.
9. Provide an approved table for the mixer, it should not be on the floor.
