    Correction: Ensure employees are properly sanitizing cleaned equipment and utensils before they are reused, through routine monitoring of solution temperature and exposure time for hot water sanitizing, and chemical concentration, pH, temperature, and exposure time for chemical sanitizing.
  • Critical: Food - Potentially Hazardous Food - Hot Holding* (corrected on site) (repeated violation)
    Observation: Marinara hot holding at improper temperatures.
    Correction: Hot hold potentially hazardous food at 135°F or above to inhibit the growth of harmful bacteria. Operator turned up the unit during the inspection. Operator agreed to discard after 4 hours or heat to order.

    Observation: The prepared ready-to-eat (RTE) wings and ham in the refrigeration unit is not properly dated for disposition. No date marking system was veiwed in pplace during the inspection.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria. Operator agreed to date mark during the inspection.

    Observation: There was no temperature measuring device located in the prep unit.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Sanitizing Solutions, Testing Devices (repeated violation)
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: The nonfood contact surface of the outside of most equipmet the inside of refrigeration has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Correction: Operator purchased sanitizer during the inspection HD verified it was at proper concentration.

    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. No self closure device on the back door.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Soap was not provided at the hand washing lavatory in the front handwashing station.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands. Operator provided saop during the inspection.

    Observation: No disposable towels were provided at the hand washing lavatory in the back preperation area.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials. Operator provided paper towels during the inspection.

    Observation: Floors, walls and ceiling in the kitchen noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner. A good detail cleaning needed along with a cleaning schedule to maintain the facility.

    Observation: Flies were/was noted in the establishment
    Correction: Prohibit all live animals on the premises of the food establishment. Animals carry disease-causing organisms and can transmit pathogens to humans through direct and/or indirect contamination of food and food-contact surfaces. Animals shed hair continuously and may deposit liquid or fecal waste, creating the need for vigilance and more frequent and rigorous cleaning effort.

A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Observed methods are not being used to control pests. Flies prent throughout the facility.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required. Operator is in the process of purchasing fly light for the facility.

Leaving contact information for the store manager to send information concerning training employees on wash rinsing and sanitizing food contact surfaces and that they have test strips to measure proper concentration. Leaving handouts for wash rinse and sanitizing and employee health information for the operator.
  • Critical: Demonstration of Knowledge* (corrected on site)
    Observation: The person in charge failed to explain the correct procedures for cleaning and sanitizing utensils and food-contact surfaces of equipment. Person in charge could not demostrate haw to wash rinse and sanitize food contact surfaces.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled. Asked the operator to fill the three compartment sink with water using leach the Health department showed staff how to mix the proper concentration during the inspection. Discussed the issue with the store manager the manager to email the Health Department with documentation that all staff were trained on hoe to properly wash, rinse, and sanitize food contact surfaces. Had staff wash, rinse and santize food contact surfaces during the inspection.

Discussed compliant with the GM. Operator agreed to review handwashing procedures with all staff. Operator stated that they usually have one person taking money and one making the pizza. I let him know this is multiple complaints about the same issue. Operator agreed to discuss procedures with all staff.

    Observation: Garlic butter and marinara hot holding at improper temperatures.
    Correction: These are commercial products so they have to be 135 in 2 hours and maintained at 135 degrees. Also discussed not saving the product from one day to the next to the facility capabilities to reheat the product to 165. Manager discarded the product during the inspection and adjusted the heat up on the hot box.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
