    Observation: The nonfood contact surface of the baseboards have accumulations of grime and debris. Also the air vents in the ceiling and above the pizza oven.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Plumbing System Maintained in Good Repair
    Observation: The hand sink basin is slow to drain.
    Correction: Plumbing systems and components shall be maintained in good repair.
  • Outer Openings - Protected
    Observation: Observed holes in walls by the coving at the hand sink and in the rest room that could allow entry of pests.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Physical Facilities in Good Repair (corrected on site)
    Observation: There was damage done to the ceiling in the restroom, there is a wooden board covering the hole. PIC has contacted a repair person.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Critical: Toxics - Conditions of Use* (corrected on site)
    Observation: The Redi San RTU is not being used in accordance with law or the manufacturer's use directions. Use a chlorine sanitizing bucket to sanitize prep tables.
    Correction: The HAZARDOUS PRODUCT must be used in accordance of law and in a manner that does prevent the contamination of food, equipment, utensils, linens or single service items.

Discussed with operator:
1. seal the gaps at the front door
2. provide a thin diameter probe thermometer for taking temperatures of thin foods such as pepperoni prior to opening for business.
This facility is in substantial compliance and is eligible for a permit from the Virginia Department of Health. The Virginia Department of Health permit will be issued to operator upon receipt of a copy of the certificate of occupancy from the Building Department.

Site visit made to new facility to observe construction progress.
Discussed with operator:
1. ensure the handsinks are equipped with handwashing signs
2. as the mop sink has soap and paper towel dispensers, recommend installing a "mop sink only" signage
3. all floor/wall junctures will be caulked or coving installed
4. ensure the counters are smooth, easily cleanable and non absorbent
5. ensure the bare wood ledge at the prep table is smooth, easily cleanable and non absorbent
Prior to reinspection, ensure all construction equipment is removed from facility and a thorough cleaning of all equipment and the floors, walls and ceiling is completed. A reinspection is scheduled for Tuesday, September 23, 2014.

Site visit made to new facility to ensure that only one handsink will suffice for this facility. There is a limited menu of pizza, mainly sold by the slice, toppings for the pizzas and drinks. Paper plates will be utilized so there is minimal washing, rinsing and sanitizing of equipment at this facility.
Discussed with operator:
1. install an air gap under the 3 compartment sink
2. time as a control will be utilized for the pizza for up to 4 hours - any pizza left over after the 4 hour period will be discarded. Once a slice of pizza is ordered, the already cooked pizza, which will be held on shelving next to the pizza oven, will be reheated for immediate service. Facility will not be reheating for hot holding. A copy of the Time as a Public Health Control fact sheet and a copy of Time As A Public Health Control Procedure has been sent to the operator.
3. operator will submit a better drawing of the lay out of this new facility to the Health Department in order for the plan review process to continue.
4. it appears, from the layout of this facility that only one handsink will be required.
