Observation: The 3 compartment sink is no longer sealed to adjoining equipment or walls.
Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

Observation: The door gasket on the reach in cooler is in poor repair.
Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

Observation: The following equipment was observed in a state of disrepair and damaged: 1. the interior of the cabinet under the rinse sink 2. the door handle on the reach in cooler 3. the door on the ice machine.
Correction: Repair/replace the equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.

Observation: Surfaces of the interior of the ice machine were observed soiled with accumulations of grime and debris.
Correction: Clean the surfaces of equipment at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

Observation: Plumbing connections under the sanitizer basin of the 3 compartment sink are leaking.
Correction: Plumbing systems and components shall be maintained in good repair.
- Physical Facilities in Good Repair (repeated violation)
Observation: The following areas are not maintained in good repair: 1. the floor tiles throughout the back of the kitchen, bathroom and by the door entering the establishment 2. the coving and wall in the restroom 3. the wall in the bathroom (hole).
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

The above observations and the following were discussed with the person in charge: -Please replace the ceiling tiles over the 3 compartment sink with tiles that are smooth and easily cleanable. -Employee health policy observed. -All in use utensils are being washed, rinsed and sanitized or changed out at least once every 2 hours. Lots of good handwashing and glove use observed during this inspection, great job by all staff members! Observation: Food stored on the floor or food stored less than 6" above the floor. Observed soda boxes stored on the floor by the dry storage shelves.;Elevate food storage onto approved shelving with minimum 6" legs or casters.
- Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
Observation: The nonfood contact surface of the tape used on the door handles of the reach in cooler is not corrosion resistant, nonabsorbent, and/or smooth.
Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
- Equipment - Fixed Equipment, Spacing or Sealing
Observation: The rinse sink is not properly sealed to adjoining equipment. The rinse sink has a large gap along he sides allowing for water to leak into the cabinets below. This is also allowing for the collection of food debris in the cabinet below the sink.
Correction: Seal the rinse sink to adjoining equipment since it is exposed to spillage or seepage.
- Equipment - Good Repair and Proper Adjustment (repeated violation)
Observation: The cabinet under the rinse sink in the front of the kitchen was observed in a state of disrepair and damaged due to the leak around the seal of the sink. The ice scoop stored on the ice machine observed to be cracked/chipped along the bottom. The handles of the reach in cooler observed to be broken, tape is being used to keep the handles in place.
Correction: Repair/replace the equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
- Equipment - Non-Food Contact Surfaces and Utensils (corrected on site)
Observation: The interior of the ice machine that is in contact with non-potentially hazardous food items was observed soiled with accumulations of grime and debris.
Correction: Clean the surfaces of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

Observation: The nonfood contact surface of the cabinet below the rinse sink has accumulations of grime and debris.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: The following areas are not maintained in good repair: 1. broken floor tiles observed by the side door of the kitchen 2. broken wall/floor tile observed under the juice machine 3. holes in the walls in the bathroom behind the door and by the floor molding.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Mops - Drying Mops (repeated violation)
Observation: Mops not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

- Critical: Backflow Prevention Device, When Required* (repeated violation)
Observation: Observed the pre-wash spray hose extend below the flood rim level of the sink basin at the 3-vat sink.
Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
