The above observations and the following were discussed with the person in charge: -Employee health policy observed. -All in use utensils are washed and sanitized every 4 hours. -Replace the door gasket on the 2nd door of the 3 door reach in cooler. -Replace the broken thermometer in the reach in cooler in the front of the kitchen. Please notify the health department once the handsink in the back of the kitchen has been repaired. - Critical: Handwashing Lavatory*
Observation: There are not an adequate number of handwash lavatories on site. (See 12 VAC5-421-2280) The handsink in the back of the kitchen is not operational leaving the facility with only one handwashing station in the front of the kitchen away from the food prep area in the back. The PIC stated they have ordered parts to repair the handsink but did not have information as to when the repairs would be made.
Correction: Install/repair additional lavatories to increase the employee's opportunity to wash hands routinely. Prior to installing additional plumbing fixtures the permit holder must apply and gain approval through the necessary regulatory agencies.

Observation: The handsink basin in the front of the kitchen is slow to drain.
Correction: Plumbing systems and components shall be maintained in good repair.
- Ventilation Systems - Cleaning Ventilation Systems - Nuisance and Discharge Prohibition
Observation: Intake and exhaust air ducts over the food prep table in the front of the kitchen (behind the sign) are not clean.
Correction: Clean intake and exhaust air ducts and change filters so they are not a source of contamination by dust, dirt and other materials
- Mops - Drying Mops
Observation: Mops not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
- Critical: Toxics - Storage of Toxic Containers* (corrected on site)
Observation: Chemicals are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed a soap bucket stored on the prep counter, a chemical spray bottle stored with condiments and bags used to hold pretzels and a chemical spray bottle stored on the sugar by the reach in cooler in the back of the kitchen. All chemicals were moved to the designated area during the inspection.
Correction: Chemicals must be located in an area that is not around food, food contact surfaces, equipment, utensils, linens or single service items.

- Food Storage Containers - Identified with Common Name of Food (repeated violation)
Observation: Unlabeled food containers. Observed a bulk container of baking soda stored without being labeled and a container of sugar stored in the back of the kitchen without being labeled.
Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.

Observation: The food contact surface of the trash bag is not durable, nonabsorbent, easily cleanable, resistant to pitting. Observed pretzels stored in trash bags in the reach in freezer.
Correction: Replace the trash bags prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.

Observation: The ice scoop stored by the ice machine was observed in a state of disrepair and damaged.
Correction: Repair/replace the ice scoop to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.

Observation: The door gasket to the 3 door reach in cooler is in poor repair.
Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
- Plumbing System Maintained in Good Repair (repeated violation)
Observation: The plumbing connections under the 3 compartment sink are in poor repair. Observed standing water/butter in the funnel connected to the pipes under the 3 compartment sink. PIC stated that the water in the sinks must be drained very slowly or the pipe will over flow onto the floor.
Correction: Repair and maintain all plumbing components and fixtures.
