This visit was conducted to do a Risk Factor assessment.
3 vat sink properly set up and sanitization method correctly explained, and sanitizer in 3 vat sink tested within required limits.
Health Policy provided and reviewed.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. raw chicken stored above dough in walk in coooler
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc.
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: The raw animal food on display is stored in a manner that may cause cross contamination: raw chicken above dough in walk in cooler
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be displayed to prevent cross contamination. Maintain separation.

    Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: mildew buildup on walls and ceiling of walk in cooler
    Correction: rust builld up on floor of walk in cooler
  • Floor , Wall & Ceiling / Design
    Observation: The floor in the toilet roomoes not meet the standard of: smooth, and easily cleanable.
    Correction: Wall, floor, and ceiling surfaces shall be smooth, durable, easily cleanable, and non-absorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, and toilet rooms.
  • Physical Facilities Good Repair
    Observation: Observed that the following are not maintained in good repair:
    1. rear exterior door is in poor repair inclduing the door frame
    2. the cold water knob for the mop faucet is missing.
    3. The light fixture for the inner walk in cooler (formely the walk in freezer) is damaged,
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
