This visit was made to conduct a routine food safety evaluation. The bathroom is still in need of a self-closing door. It still also needs a vent cover. There is no CFM present. The person in charge was told that a CFM is not needed for the grocery side, but that they will need to keep a temperature log for the dairy and egg cooler, as well as records of an employee health and hand washing policy. *Repeat violations are subject to civil penalty*

This visit was made to conduct a change of ownership evaluation. No open foods prepared or handled. Provided cold holding temperature logs for recording of unit containing Time/Temperature Control for Safety (TCS) foods. The following items shall be corrected within 30 days: 1. Provide a self closing mechanism for the employee rest-room 2. Provide a vent cover for vent located in the rest-room 3. Provide a light cover for the fixture in the rest-room 4. Replace burnt out lights for the ice cream chest freezer, True upright refrigerator, 3-door upright refrigerator, and the True 2-door "Coca-Cola" refrigerator Approved for change of owner recommend issuance of Alexandria Health Department permit. This visit was made to conduct a change of ownership evaluation. Prior to opening the following items require correction: 1. The floors throughout the grocery store are heavily scratched and scored. Repair damaged floor to provide a smooth and easily cleanable surface. The floor and wall juncture in food establishments shall be coved and closed to no larger than 1 millimeter. 2. Ceiling tiles above food storage shelving throughout grocery stored are constructed of porous material and many of them have been damaged with water and now stained. Replace ceiling tiles with approved tiles that are smooth and easily cleanable. 3. Walls throughout grocery store are either constructed of exposed dry wall or they are pitted and damaged. Repair all walls throughout grocery store to provide a smooth and easily cleanable surface. 4. The exposed wood for the back door and at the front counter shall be sealed to provide a smooth and easily cleanable surface. 5. Light fixtures above grocery food aisles, in the 3-door upright refrigerator and in the 3-door upright freezer are not shielded or protected. Light bulbs shall be shielded, coated, or otherwise shatter-resistant. 6. One of the glass doors for the 3-door upright freezer is shattered and shall be replaced. 7. This food establishment is connected to 807 Pendleton via a proposed pass through hallway. Make sure when remodeling the pass through, final finish surfaces are smooth and easily cleanable. 8. This food establishment uses the same commercial hot water heater located on the 807 Pendleton side. Make: Rheem Model: EGS - 12 - 85 Input: 12 KW Required recovery rate for both the grocery side and the fast food side of the food establishment is 87 GPH. The current hot water heater is 29% insufficient. An 18 KW input will be required in order for both food establishments to have a sufficient recovery rate. 9.
