Today I conducted a follow up inspection and discussed with management the following:
1. Management should reiterate the 5 major risk factors associated with foodborne illness with all staff frequently: Employee health policy, Personal hygiene, Contaminated equipment/utensils, Time/Temperature relationships for food and Unapproved food sources.
2. Management should continue to calibrate all food thermometers in the ice water bath as often as needed to ensure accuracy when taking food temperatures. The ice water bath is a cup completely filled with ice and some cold water and the thermometers should read 32f. The thermometers should be washed, rinsed and sanitized BEFORE and AFTER each use.
3. Management should continue to set up the 3-vat sink each morning to wash, rinse and sanitize. WASH in hot, soapy water. RINSE in hot, clean water and SANITIZE in cold water (65f-75f) and Quaternary ammonium at a concentration of 200-400ppm as per manufacturer's specifications. Change the water at the 3-vat sink and wet towel buckets at least once every 2 hours or more often as needed.
4. Management should continue to clean and sanitize all floors and walls especially in the dry storage area.
5. Management will continue to maintain weekly services from the licensed pest control company. Last services were 3/4/2016, 3/11/2016 and 3/18/2016. Technician has observed less issues with pests.

  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation (corrected on site) (repeated violation)
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: 1. Wall mounted fan at the dish machine used to sanitize 3D glasses.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues. DISCUSSED WITH MANAGEMENT. MANAGEMENT SHALL BE INSTRUCTING STAFF TO CLEAN AND SANITIZE EVERYTHING THEY SEE AND TOUCH DAILY TO REMOVE VISIBLE DEBRIS AND RESIDUE.
  • Physical Facilities Good Repair
    Observation: Observed that the kitchen ceiling panels in some locations are not maintained in good repair. Observed small holes in the walls inside the mop sink closet. Observed openings in the ceiling panels in the kitchen areas where large pipes are coming through.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. DISCUSSED WITH MANAGEMENT AND PRODUCTS HAVE BEEN ORDERED TO SEAL THE GAPS. CONTINUE TO REPLACE MISSING CEILING PANELS AS NEEDED. CONTINUE TO SEAL OPEN ENTRY POINTS IN THE CEILING AND WALLS TO PREVENT ENTRY OF INSECTS.

There is a CMA Dish washing machine in the back, on the same wall as the 3-Vat sink. Senior Manager Juliana Levitt stated it is used only for IMAX Theatre Glasses, and not for food-related items. Also present were Facilities Manager John Marzec and Supervisor Tim Stevenson.
  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: Ice Scoop in ice bin behind service line with handle in ice.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. EHS demonstrated proper storage to employee and discussed with PIC.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following units are damaged: True 2-Door Upright Cooler on back wall of storage area, and, on service area back wall: True 2-Door Upright Cooler (far right), True 2-Door Flat Top Freezer ("Fries"), True 2-Door Upright Cooler (Hot Dogs)
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. REPAIR ALL DAMAGED DOOR GASKETS.

  • Food Storage Containers, Identified with Common Name of Food (repeated violation)
    Observation: Observed unlabeled spray bottle with clear liquid. According to CFM, water which is used for pretzels was being stored in the spray bottle. CFM agreed to label.
    Correction: ALL food and ingredients removed from their original container and stored in other food containers, squeeze bottles or "shakers" shall be labeled properly using the common name of the food (if the food or ingredient is not easily distinguished). For example, all cooking oils, salt, sugar, flour, starch, spices, herbs, etc. shall be labeled.
  • Plumbing / Maintained in Good Repair
    Observation: The sink basins at the handwashing sinks (by 3-vat, and closest to 2DR refrigerator (commercial beverage) are slow to drain. Discussed with CFM.
    Correction: A plumbing system shall be maintained in good repair.
06/25/2013Routine
The purpose of this visit was to conduct a complaint inspection based on a complaint received by FCHD. The complainant stated that they had observed a cockroach.
Certified food manager provided pest logs for the routine weekly treatments.Upon inspection of the facilities, no pest activity was observed.
The complaint is not confirmed.
