1: 2-102.11: C: The person in charge could not demonstrate required knowledge. INSPECTOR NOTES: Several managers were questioned regarding safe holding temperatures for PHF's and about proper warewashing procedures and use of the Kayquat sanitizer. They were not sure of proper cold holding temperatures or about the correct way to sanitize. It is recommended that they have refresher courses.
4: 2-401.11: C: Food employee is eating, drinking, or using any tobacco where the contamination of exposed FOOD; clean EQUIPMENT, UTENSILS, and LINENS; unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES; or other items needing protection can result. INSPECTOR NOTES: Manager observed drinking from an uncovered berverage in a food prep area.
14: 4-602.11.(C): C: Potentially hazardous food contact surfaces are not cleaned with proper frequency. At least every 4 hours. INSPECTOR NOTES: There is no physical means of tracking frequency of equipment in constant use however when asked when trays had last been washed and sanitized it was thought to have been around 4 and 3/4 hours prior. COS.
14: 4-703.11: C: Manual and/or mechanical methods of sanitizing incomplete. INSPECTOR NOTES: There are no directions for use of the sanitizer for ware washing. Trays were just dipped in the sanitizer and not left for any contact time. Please post directions at the sink and train employees in the proper procedure. COS
22: 3-501.19.(A): C: Food held with time as the only control was not properly marked, was not properly discarded. INSPECTOR NOTES: The clocks for determining discard time were not being used and one clock is broken. PHF's in the danger zone were discarded.
38: 2-402.11: N: Food Employees not wearing effective hair restraints. INSPECTOR NOTES: Managers working with food without hair restraints and a food worker with her visor under her hair.
39: 3-304.14.(B).(2): N: Wiping cloths improperly stored or used. INSPECTOR NOTES: Wet wiping cloths are left on the counter and not returned to the sanitizer bucket after use. The had no sanitizer residual on them. The sanitizer buckets had no sanitizer residual in them. COS
43: 4-903.11.(C): N: Single service/ single use items improperly stored. INSPECTOR NOTES: Takeout containers in the shed are not stored at least 6" off the floor.
46: 4-302.14: N: No chemical test kit available. INSPECTOR NOTES: No test papers for the KayQuat sanitizer.
