Manager stated that the date was the day it was put in the freezer, and she did not update the date to 10-23-15 when she placed it in the cooler. COS - packages redated.
KDA_4_60111A - --In drawer, a knife with dried food debris, was stored as clean. Corrected on site (COS) - knife was put at sink for cleaning.
KDA_4_60213 - --There was a build up of mold on the both milk cooler lid seals.
KDA_Defacto - --Cold Holding: Walk in cooler - butter 35.8F. Milk cooler (east) - 37.6F. Milk cooler (west) - 37.9F. Freezer ok. --Hot Holding: Warmer - taco meat 173F. --NOTE: When testing dish machine sanitizer (chlorine), no sanitizer registered on test strip. Manager called maintenance, and they were working on machine during inspection.
