KDA_6_20215A3 - --There is an exterior south door that has a 1 in. gap that has rags stuffed in it.(The door appears to be bowed in where it has been broken into). This allows for roaches to hide from cleaning/pesticides. Advised to fix. KDA_6_501111A - --New violation-There were 2 live roaches by the large pepsi cooler, 2 live roaches crawling out of the onion bucket, 1 live roach under the diswasher, 1 live roach under the make table, 1 live roach in a kitchen shelf, 1 live roach behind a food prep table, 20 live roaches in the trim behind the small pepsi cooler, and several casings on the floor in the kitchen. Pest control last serviced the establishment on 9/30/15. COS-facility will close. KDA_6_501112 - --Repeat violation-There are too many to count dead roaches on the floor in every room of the establihsment. Advised to clean.
