- KDA_2_30114H - --Employee was prepping raw carrots and took off gloves to put ready to eat cheese in a container, she then walk to the walk in cooler and put the cheese in there. Employee then proceeded to come back and put on gloves and start prepping carrots with out handwashing. COS-Employee discarded gloves and washed hands. Carrots were washed.
- KDA_2_40111 - --Sitting on the make table, where employee was prepping raw carrots, there was an open coffee cup with coffee in it. COS-employee moved the cup.
- KDA_3_30211A1a - --In the walk in cooler, there was a box of 7.5 dozen eggs, a box of 5 dozen medium eggs and one 18 count carton of eggs sitting above open an milk carton, package of bread and two cartons of sour cream. COS-Employee moved the eggs to the bottom shelf. There was no leakage from the eggs. Issued a FWL.
- KDA_3_50118A1 - --In the walk in cooler, there was a container of refried beans with a date of 9/10/15. They were held over 7 days. COS-beans were voluntarily discarded.
- KDA_4_60111A - --There were two metal pans on a shelf below the prep table, with sticker residue on the outside of them. COS-employee set these at three compartment sink to be scrubbed.
- KDA_4_60111C - --The commercial mixer, has white powder on the non food contact surface.
- KDA_6_50111 - --In the kitchen area, there are loose ceiling tile and one stained ceiling tile above the stove.
- KDA_6_50112A - --In the walk in cooler, there is an accumulation of dust on the ceiling and the fan box.
- KDA_Defacto - --Ambient temperature of walk in cooler was 40.8F. --On the stove top, a pot of cooked on site shredded chicken was 203.4F.
