- KDA_3_50116A2 - --Cooked on site philly meat, 43F, 46F. Ambient of walk in was 44.2F. Person in charge said the compressor vibrates and the temperature gets turned down. COS-temperature control was turned down. 15 minutes after it was turned down the ambient was 37.6F. Second reach in make table, corn dogs 47.1F, chicken strips 49.2F. Person in charge said these were stocked from the walk in cooler at 11:00AM. The make table reach in, is open and closed a lot. COS-left to cool down. Ambient of the reach in make table was 30.6F. On the make table, single serve gravy 50.6F. Person in charge said it had been out since 11:00AM. COS-gravy was discarded.
- KDA_3_50119B2 - --When inspector walked in, employee was changing time on laminated card for time as control items. He was marking beginning time as 2:00PM. Employee said sliced tomatoes and lettuce leaves had just been gotten out of walk in cooler and he had forgotten to change the time. When inspector took temperatures and questioned the person in charge, he said that that the sliced tomatoes were put out around 1:30PM and lettuce leaves were put out around 1:00PM. COS-laminated card was corrected with the correct beginning time.
- KDA_4_20211A2 - --Above the three compartment sink, there were seven metal pans that were pitted along the food contact surface. On the green shelf there were six containers that were pitted along the food contact surface area. COS-items were discarded.
- KDA_4_20311 - --Food thermometer was not accurate, showed person in charge how to calibrate it. There was not a tool available to move the screw. Person in charge said he will order new ones.
- KDA_4_60111A - --Above the three compartment sink, there were two metal pans stored as clean with dried on food debris. Stored as clean on a green shelf, with fried on food debris was one metal ladle, four metal lids and five plastic containers. COS-items were moved to three compartment sink for washing.
- KDA_4_60111C - --The grate on the chest freezer by the grill, has an accumulation of dust. The vent hood over the grill has an accumulation of dust. Fan covers in the walk in cooler and the walk in freezer have an accumulation of dust.
- KDA_4_60211E4 - --Behind the pop nozzles and the metal ledge on the pop machine in the customer self serve area, there is brown and black mold.
- KDA_6_20215A3 - --There is a small gap in the right hand corner of the back exit door.
- KDA_6_30114 - --There were no hand washing signs in the men's or women's restrooms. COS-Left signs.
