  • 4-302.14 - --The owner does not have chlorine test strips available. She uses chlorine as a sanitizer on her dishes. She said she has found a place to purchase them, but has not yet gone to the supply store and done so.
  • KDA_3_30211A1b - --In the walk-in-cooler there is a package of raw sausage, a package of raw beef, and a box of raw shelled eggs being store don a shelf directly above a pallet on the floor that has ready-to-drink soda pop products in cans on it. Corrected on Site (COS) - the owner moved the raw products.
  • KDA_3_30212 - --There are unlabeled squirt bottles that have been identified as ketchup, mustard, and mayo. The owner said she has labeled them three times but the label keeps coming off and asked for suggestions on how to get a label to stay on. Made suggestions based off what other establishments have tried doing.
  • KDA_3_30412B - --A coffee cup is being used as the scoop in the ice. The handle has fallen down in direct contact with the ice.
  • KDA_3_60311B - --The establishment has a consumer advisory posted on the walls and in the menus "Consuming undercooked meat or eggs can be hazardous to your health." They do not have a disclosure stating which items they offer cooked to order. The owner said previously they will do eggs to order. She said that she has a new sign for the walls being made, but it is not there yet because she does not personally have a computer to use to make it.
  • KDA_4_10111A - --In the cooler in the kitchen there is a package of raw bacon in a sterilite multi-purpose container with no markings indicating it is a food grade container. Part of the bacon is sticking out of the package and is in direct contact with the plastic of the container. COS - the owner put wax paper between the bacon and the container. There are sliced potatoes in a gray sterilite multi-purpose container in the kitchen. This container has no markings indicating it is food grade and the potatoes are in direct contact sitting in water in the container. COS - the owner put the potatoes into a stainless steel container. The ice is in a sterilite container with no markings indicating it is a food grade container. The ice is directly in the container. The owner is going to purchase a new container for the ice.
  • KDA_4_60111A - --The ice container has a sticker on the outside of the container that was identified as remaining from when the container was originally purchased.

  • 3-304.15(A) - --The cook had a glove on to use with ready-to-eat buns. She then took the glove off, put it on the counter in the kitchen, and left the kitchen to grab an item from the walk-in-cooler. She returned to the kitchen, grabbed the glove off the counter and one from the boxes of gloves, washed her hands, and put the gloves on. She then used the gloves in handling ready-to-eat food. COS - explained to her that if her task is interrupted and she needs to remove her gloves, she needs to start with new gloves rather than reusing a previously used glove. She began replacing gloves after tasks.
  • 4-302.14 - --The owner does not have chlorine test strips for a food establishment, she only has chlorine test strips for a pool which do not offer the proper readings needed for the sanitizer water. Explained the 1-2-3 method (1 cap of bleach to 3 gallons of water) to her. She will be getting test strips in.
  • 4-102.11(B)(1) - --In the walk-in-cooler there are three bags of squash. The squash is being stored in direct contact with the plastic of three grocery bags (Dillons, Walmart bags).
  • KDA_2_30114I - --The cook put gloves on and handled raw hamburger while putting them on the grill with her gloved hands. She then removed the gloves, handled/washed out some dirty dishes, and put fresh gloves on prior to handling buns with her gloved hands. Corrected on Site (COS) - explained the hand washing violations to her and when she needs to wash her hands. She began washing hands between pairs of gloves. Did not require bun to be discarded.
  • KDA_3_10111 - --In the walk-in-cooler there is an opened gallon of milk with no date on it. The owner said she is unsure when this was opened, as she never keeps opened milk in the walk-in-cooler and doesn't know who put it here. COS - she discarded the milk.
  • KDA_3_20111A - --There are 20+ dozen brown farm eggs in the kitchen and in the walk-in-cooler. The owner said she was not aware she was not allowed to use farm eggs in her establishment. COS - these farm eggs were all removed from the establishment during the inspection. They are being released to the owner for personal use.
  • KDA_3_30111B - --The cook grabbed three ready-to-eat biscuits with her bare hands to put them in a container of biscuits. COS - stopped her and explained the violation. Biscuits were discarded. The same cook later handled two pieces of bread with her bare hands and put them into the toaster. COS - explained that toasting is not a thorough cooking process and that the bread may not be handled with her bare hands. Bread was discarded.
  • KDA_3_30211A1a - --In the small black Galanz cooler there are raw shell eggs on a shelf directly above raw, chopped green peppers and onions. COS - the peppers and onions were moved.
  • KDA_3_30211A1b - --In the white Crosley walk-in-cooler there is a container with raw hamburger patties being stored directly above sliced cheese in a drawer. COS - the cheese was moved.
  • KDA_3_30212 - --There are unlabeled squirt bottles that were identified as being ketchup, mustard, and mayo.
  • KDA_3_30412B - --There is a cup being used as a scoop in the ice tub. There is no handle/designated 'grab here' location on this cup. The cup is stored in direct contact with the ice.
  • KDA_3_30414B1 - --There is a wet wiping cloth sitting on top of the drinks cooler, rather than being stored inside of a chemical sanitizer.
  • KDA_3_50118A2 - --In the cooler in the kitchen there is an open gallon of milk with no date marking on the container. The owner said the milk was opened on 8/17/15. COS - she dated the container.
  • KDA_3_60311B - --The establishment has a consumer advisory posted on the walls and in the menu "Consuming undercooked meat or eggs can be hazardous to your health." They do not have a disclosure stating which items they offer cooked to order. The owner said she will do eggs to order.

  • KDA_4_204112A - --The thermometer inside of the Crosley cooler is in the back of the cooler, rather than in the front where it will be warmest.
  • KDA_4_50111A - --The base of the cabinet below the hand sink is broken and chipped.
  • KDA_4_60111A - --There are stickers on the outside of the biscuit container and the ice container. These were identified as remaining from the original purchase manufacturer's label of the container. The owner said that these were cleaned prior to use, but the sticker was not removed during the cleaning process. Explained to her that the stickers may not be left on the containers.
  • KDA_4_60111C - --There is dust on the blades of the fan above the grill.
  • KDA_4_90311B - --There are non-inverted dishes throughout the kitchen and waitressing area. These include plates, containers, and bowls.

  • KDA_5_20511B - --The hand sink in the kitchen has a stopper in the bottom of the sink and water standing in it. There are dirty dishes piled in this sink. Upon checking the kitchen, all sinks appear to be in use for dish washing. Upon speaking with the owner, she said she was scrubbing the dishes out in the hand sink before taking them to the 3 vat sink to wash/rinse/sanitize. COS - she emptied the hand sink of dishes and unstopped it so that hands could be washed. Explained to her that the hand sink always needed to be accessible.
  • KDA_5_20515B - --The hand sink in the men's restroom has a leak from the faucet while turned off. The spray head on the 2 vat sink where the hand sink is located leaks water when the water is turned on.
  • KDA_5_50117 - --There is no covered trash can in the women's restroom.
  • KDA_6_10111A - --The shelves in the walk-in-cooler and the floor are made of unsealed wood. The owner said she has someone coming in to give her a quote on how much it would be to redo these.
  • KDA_6_30112 - --There are no paper towels available at the hand sink in the kitchen. COS - the owner said her paper towel dispenser broke and hasn't been replaced yet. She provided paper towels to use for the time being.
  • KDA_6_50119 - --The door to the women's restroom was propped open during the inspection. Cleaning and maintenance were not being performed.
  • KDA_Defacto - --Cooking Hamburger - 192.7 F --Dishes done during the inspection were sanitized in 50ppm chlorine. --Galanz Cooler: Eggs - 41.8 F Crosley Cooler: Milk - 43.9 F Walk-in-Cooler: Eggs - 37.1 F --Let the owner know that if they hook up the ice machine in the side storage area, the ice machine must be indirectly plumbed. --Rival Crockpot: Liver & Onions - 180.4 F --There are three packages of Brisket from a meat locker in the walk-in-cooler. All three are labeled "Not for Sale." The owner said that these belong to an employee who forgot to take them home last night and are not being used by the establishment.

It was a very unclean place and not once did the person come back after food was brought.

Many young girls handling the kitchen and it was apparent that they were not supervised.

BBQ'ed food was being prepared outside in a smoker east of the café in an unpaved parking lot.
