KDA_3_30611 - --At the self-serve drink area there is an open bowl of ice that has no protection from contamination.
KDA_3_50118A2 - --In the pizza make table there are open containers of ready-to-eat beef and sausage that have no dates on them. The owner said that she opened these on Saturday (8/29/15). Corrected on Site (COS) - she dated the containers.
KDA_4_90311B - --There are non-inverted and uncovered pizza pans and plastic containers in the kitchen.
KDA_4_90311C - --Throughout the kitchen, storage room, and in the self-serve area there are single-use Styrofoam plates, paper plates, pizza boxes, and plastic cups being stored outside of their original protective packaging, neither covered nor inverted.
KDA_4_90312A - --There are packages of Styrofoam plates, paper towels, and napkins being stored under a dish rack of lids that have food residue on them and were identified as dirty lids that are to be cleaned.

The owner said these are purely personal use. Let her know that it is best to label her personal use items as personal use only. --Make Table: Sausage - 38.7 F Cooler under Make: Cheese - 42.8 F White Cooler: Ham - 39.4 F --Ownership verified. --Thaws as part of the cooking process. --There are lids on a cart near the make table that have food residue on them. The owner said these are dirty lids that will be washed. --There is a wiping cloth in the sink in soapy water. The owner said she had made this to clean a toy for one of her grand kids, it's not for establishment use.
