KDA_3_20111C - --Gar Hole All Purpose Seasoning labeled manufactured for Poverty Flats Spice Company, Fort Scott, KS 66701, 620-547-2335 misbranded as the labels lack the address of the manufacturer / distributor. Poverty flats is not in the current phone book. As per employees, it is delivered in person. Unknown where it is manufactured and/or warehoused. Product pulled from retail sale. KDA_4_20216 - --A wooden knife block is in the deli with knives stored in it, these are prohibited. KDA_5_20515B - --The men's restroom handsink faucet is missing the screen on the water inlet. At the mop sink, an atmospheric vacuum breaker is built into the faucet. A wye valve is hooked to the faucet. On one side, a hose goes to a chemical dispensing unit that has a built in backflow prevention device. Nothing is hooked to the other side. A hose must never be hooked to the empty side until the breaker is replaced or repaired. KDA_6_20215A3 - --On the outdoor smokehouse, gaps are at the bottom corners of the exterior door of about one inch. In dry storage, north of the produce prep area, a gap of about 3/4 to one inch is at the bottom of an exterior door. All gaps are large enough to allow pest entry. KDA_6_20215D - --On the outdoor smokehouse, a hole of about two inches is in a window screen on the north side, large enough to allow pest entry. KDA_6_20216 - --On the outdoor smokehouse, there is a hole in the west wall of about one inch, a gap of about one inch at the southwest upper corner, and a 1/2 inch gap at the floor / wall juncture at the northeast end.
