  • KDA_3_30211A1b - --On the retail floor there is raw pork bacon in a cooler directly next to/in contact with fully cooked turkey bacon. There is also a package of raw pork bacon that is sitting on top of the fully cooked turkey bacon in the back. The manager moved the bacon in the back into the correct stack. Told him there needs to be a separation between the raw pork bacon and the fully cooked turkey bacon so that they are not in contact with one another. He said they will probably move the turkey bacon to another shelf.
  • KDA_4_10111A - --There is a Sure Sprayer chemical sprayer being used to store water that is used to spray the produce on the retail shelf. The owner said this sprayer has never been used for anything except clean water. Explained to him that the spray is only labeled for chemical use and needs to be switched out. Looking the spray up online, it is stated as being "2.0 gallon SureSpray™ Multi-purpose sprayer for Herbicides, Pesticides, Fertilizers and general cleaning." The owner called the number listed on the website and the company said that the container is not FDA approved. He is ordering a replacement container.

  • 4-302.14 - --Chlorine test strips were provided in the meat department room. They are all a light purple color instead of white. The meat department employee said that they had been dropped in water at one point. Explained to her that they turn purple when they react with bleach and that these strips that have been exposed to moisture/gotten wet will no longer give proper readings. She wrote down that new test strips need to be ordered. Explained 1-2-3 method (1 cap of bleach to 3 gallons of water) to her for the time being.
  • KDA_3_10111 - --There are strawberries in a container in the produce walk-in-cooler that are molded. COS - the produce employee discarded the molded strawberries.
  • KDA_3_30211A1b - --In the meat department walk-in-cooler there are two packages of raw pork bacon on a shelf directly above ready-to-eat cooked food products (lunchmeats). COS - the meat department employee removed the bacon. She said that the items on the shelf with the bacon were expired items that had been removed from the retail shelf, and that the items below the shelf the bacon was on were items that would go out on the retail floor. Recommending they rearrange their cooler so that the expired/damaged/etc items are not sitting on a shelf where they may contaminate good product that is intended for the retail shelves. In the retail meat cooler there is raw pork bacon stored on a shelf/on top of packages of fully cooked Turkey Bacon. COS - an employee rearranged this and moved the turkey bacon. In the meat department display cooler there is raw chorizo being stored directly next to/in contact with ready-to-eat cheese. COS - an employee moved the chorizo.
  • KDA_3_30212 - --There is an unlabeled spray bottle with clear liquid in it sitting on the retail floor. An employee said this was water used to spray the produce and keep it fresh.
  • KDA_3_30511A1 - --In the meat department walk-in-cooler there are boxes of packaged meat being stored directly on the floor. In the outdoor walk-in-freezers there are packaged frozen meats that have fallen onto the floor or have been set on the floor. There is a box of food on the floor in the produce walk-in-cooler. There are whole watermelons sitting directly on the floor by the coolers in the retail area. There is a box of corn and boxes of croutons sitting directly on the floor in the retail area.
  • KDA_4_10111A - --There is a chemical sprayer on the floor in the retail area with water inside. An employee said this water was used to spray produce and keep it fresh. Explained to her that this container was meant for use with chemicals. (Note - the chemical states that it can be used for weed killer, paints, etc)
  • KDA_4_20211A2 - --There is a knife in the knife holder in the meat department that has the end of the knife chipped off. COS - an employee discarded this knife.
  • KDA_4_50111A - --The interior walls/doors of the walk-in-cooler for the meat department have rust buildup showing on them. There are shelves on the retail floor that have the paint scraped off/rust showing on the shelves.
  • KDA_4_50111B - --The door seal (near the bottom of the meat department walk-in-cooler door) is hanging/pulled away from the door.
  • KDA_4_60111A - --There is a crockpot below the meat slicer in the meat department that has an accumulation of fallen food debris on it. The meat department employee said this crockpot is being stored as clean between uses, and that it is rarely used. She also said that it is always washed again prior to use as it tends to accumulate fallen debris from the meat slicer with where it is stored. The soaker pads near the tenderizer in the meat department have fallen dried food debris on them. COS - the meat department employee discarded these soaker pads and replaced them with fresh ones. There is a knife/cleaver in a wall holder in the meat department. These knives are all clean per the meat department employee. There is visible rust on the blade of this cleaver. COS - the meat department employee cleaned it. A knife, stored as clean in the wall-mounted knife hold in the meat department, has dried food grime at the connection between the blade and the handle. COS - this was moved to ware washing. The tenderizer blades are being stored as clean on a shelf in the meat department walk-in-cooler. There is dried food grime on one side of the blades. COS - the meat department employee set these to soaking so they could be cleaned. There is a spot of food debris on the back of the meat slicer in the meat department. The meat department employee said that it had been used today, but that it had already been cleaned. COS - she cleaned the meat slicer.
  • KDA_4_60111C - --The shelves of the produce and meat walk-in-cooler have dirt/pieces of cardboard buildup on them. There is a small accumulation of dust on the fan covers of the meat department walk-in-cooler. There is grime buildup on the light shields in the meat department room. There is dust on the interior ceiling of the dairy walk-in-cooler (directly in front of the fans). The wall-mounted knife holder in the meat department has grime/food particles on it directly where the knives slide in and out of the holder.
  • KDA_4_90311A - --There is a box of gloves and boxes of party trays being stored directly on the floor in the meat department. There are paper towels being stored directly on the retail floor.
  • KDA_4_90311C - --There are Styrofoam trays and plastic single-use party tray lids in the meat department that are stored outside of their original protective packaging, neither covered nor inverted.
  • KDA_5_50117 - --There is no covered trashcan available in the women's restroom.
  • KDA_6_10111A - --There is exposed wood around the door of the produce walk-in-cooler (it is chipped/possibly rotting from moisture exposure in places). There is exposed wood on the floor in the walk-in-cooler.
  • KDA_6_20211A - --There is an uncovered light in the walk-in-coolers in the meat department, produce department, and dairy department. The lights in the meat department have plastic tubs to shield them. The ends of these tubes that enclose the light completely inside the tube are missing.
  • KDA_6_20213A - --There is a wall mounted insect control device in the meat department. It is mounted directly above food equipment (a saw).
  • KDA_6_20215A3 - --The back door near the produce area has a narrow 1/4 to 1/2 inch gap along the bottom threshold where sunlight can be seen shining through while the door is closed. The back loading door has 1/2 to 3/4 inch gaps along the side of the door where sunlight can be seen shining through while the door is closed. The front doors have gaps around the edges of the doors 1/4 to 3/4 inch wide where sunlight can be seen shining through while the doors are closed.
  • KDA_6_30111 - --In the meat department there was no soap available at the hand sink. Corrected on Site (COS) - the meat department employee provided soap. She said the soap has a bad tendency of falling/being knocked behind the sink. Recommending a wall mounted soap dispenser to prevent this being a continual violation in the future.
  • KDA_6_30112 - --There are no paper towels available in the men's restroom.
  • KDA_6_30114 - --There is no hand washing sign posted at the hand sink in the produce area. COS - provided with a sign to be posted.
  • KDA_6_50111 - --There is a stained/sagging ceiling tile near the coolers on the retail floor. There are sagging/broken ceiling tiles in the meat department. There is exposed insulation hanging down from the ceiling in the dry storage room.
  • KDA_6_501111A - --There are 15+ flies buzzing around in the meat department room. These flies are landing on everything from the floors and walls to the food prep counters, equipment, paper towels at the hand sink, and cutting boards. Spoke with the meat department employee about this and cross contamination concerns. She said they do wipe their cutting board down with bleach water prior to use because of the flies. There are also 10+ flies in the produce area, also landing on the cutting board, inside of the produce washing sink, etc. Told employee to speak with their pest control company about options reduce/remove the flies since exposed food is worked with in this area.
  • KDA_6_50112A - --There is debris behind coolers/underneath wall shelves on the retail floor.
  • KDA_7_30111B - --On the retail shelf there is baby powder, baby oil, shampoo, Wet Ones, and Baby wipes being stored on shelves directly above Baby food. Spoke to an employee about this being rearranged.
  • KDA_Defacto - --An employee indicated that the mop water is dumped outside. Let this employee know that the sink she identified as being the sink where the mop bucket is filled appears to be available to be used as a mop sink and the mop water should be dumped here. Alternatively, it can also be dumped in the toilet. --Meat walk-in-cooler: Bacon - 41.6 F Produce walk-in-cooler: Chopped Cabbage - 41.3 F Dairy walk-in-cooler: Milk - 41.8 F Retail: 4 Window Dairy Cooler: Yogurt - 40.8 F Reduced Price Cooler: Hot dogs - 43.0 F Dairy Cooler: Cheese - 40.2 F (Note - There were cheeses in this cooler measuring 47.1 F and 48.8 F. This is not cited as a violation as the dairy employee believes the cooler was in defrost around noon and these items could be cooling, but also spoke to her about high the product was stacked and not stacking it as high, as it appears that it is stacked high enough to block the air flow) Packaged Meat Cooler: Bacon - 40.3 F Meat Display Cooler: Ground Beef - 40.3 F Produce Cooler: Chopped Cabbage - 51.8 F (Note - employees stated that this cooler was just coming off of its defrost cycle) --Spoke with lady in produce about having a trashcan with no lid on it for paper towels to be thrown away in after washing hands. Explained to her that as it is, it is difficult to throw paper towels away from washing hands without touching the trashcan, which would be considered possible contamination and require hand washing. Recommended the lid on the trashcan be removed, or a trashcan with no lid be provided. --The employee in produce indicated that when she needs to wash her knives/utensils/etc she washes them with soapy water and that is it. She said that she will sometimes use an all-purpose cleaner with bleach, but not every time they are cleaned. Upon checking the all-purpose cleaner, it is not a food grade sanitizer. Explained to the employee that it should not be used to sanitize her dishes, and that since there is no 3 vat sink in the produce area, the items needing to be washed should be washed in the 3 vat sink in the meat department room so that they may be washed, rinsed, and sanitized prior to air drying. Did not see dishwashing during the inspection. --The sink identified by the produce employee as the sink where produce is washed is directly plumbed. Food was not placed in this sink during the inspection. Explained to her that since the sink was directly plumbed, food may not be placed in this sink. Explained options (indirectly plumb the sink, have a tall container the food goes into instead of the sink)
  • KDA_KSA65_657a - --On the retail shelf there is Claritan Liquid Gels that are expired. Lot 1304977, Exp 8/14. On the retail shelf there are three boxes of Day Time Sinus Relief that are expired. Lot 3D34165, Exp 6/14. Lot 3L34116, Exp 7/15 (2).
