There were approximately 10-20 rodent droppings along the balloon weights and on the shelf in aisle 11. There were 10-15 rodent droppings along the inside ledge of the front window and in the cabinets under the registers. There is also a musty urine smell coming from the cabinets under the registers and on some of the racks at the register areas. The pest control paperwork stated that one mouse was caught in the retail area yesterday 1/18/16 and today 1/19/16. Employee stated the dead mice have been caught sporadically throughout the facility and a live mouse was spotted today 1/19/16.

  • KDA_3_10111 - --REPEAT: There were several packages/cases of RTE foods on the retail floor such as: candy, crackers, coffee, noodles, canned goods, etc. that were adulterated by rodents (teeth marks, chewed open, droppings in/on packages, urine visible on products). All food products on retail floor were voluntarily discarded per management discretion. Mark down paperwork will be sent to main office today 12/16.
  • KDA_4_60111C - --There were hundreds of fresh and old rodent droppings and nesting materials (chewed boxes,paper,towels) on the retail shelves in the establishment.
  • KDA_6_20215A3 - --REPEAT: There is a 1/4 inch gap under the right side of the back delivery door in the storage area large enough to allow the entry of pests. Recommended resizing the sweep in order to reduce the gap.
  • KDA_6_50111 - --There were several holes in the wall located such as: next to the retail reach in coolers, next to the walk in cooler in the back storage area, up approximately 12 feet on the wall in the back storage area and along the bottom of the wall close to the restroom area. Many of the holes were repaired on site by maintenance. There is a 1 inch gap between the dry wall and the concrete in the back storage area where several holes are being located once uncovered by harborage.
  • KDA_6_501111A - --REPEAT: There were approximately 6 live mice in the retail area under shelves and running into holes in the wall. There were also approximately 5 live mice inside of metal traps in the back storage area. There were fresh rodent droppings (hundreds) throughout the establishment in the storage area, under and on shelves, on and inside packages of food and single service products. The pest control company treated during inspection. Paperwork provided for establishment, the tech stated the issue has been resonating since September of last year (2014) and the first time he saw any progress was 12/1/15 and on. He will continue daily visits until the issue is resolved.
  • KDA_6_501112 - --There were approximately 10 dead mice caught in traps in the back storage room and in the retail area.
  • KDA_6_50112A - --There is fresh and old rodent droppings throughout the facility in the back storage area, under the retail shelves, under the retail reach in cooler, on the retail shelves, on and inside retail packages of RTE food, single service products and dog foods and in the back storage area. There is a musty smell in the retail area, on the shelves and in the back storage area.

  • KDA_3_10111 - --There were several cases of various RTE items adulterated (chewed through packages by rodents, droppings on and in packages from rodents) in the back storage area and on the retail shelves such as: lasagna noodles, trail mix, soup, mac/cheese noodles. An inventory list of "mark downs" will be attached.
  • KDA_6_50111 - --There are several large holes along the wall in the back storage area. Some of the holes have been filled with wool. PIC stated maintenance is coming to seal holes off entirely tomorrow 12/9.
  • KDA_6_501111A - --There are hundreds of fresh rodent droppings throughout the facility to include the retail shelves, under the retail shelves and in the back storage area. PIC stated more than 15 live mice have been disposed of in the last few days.
  • KDA_6_50112A - --There are hundreds of rodent droppings on boxes and on the floor in the back storage area as well as on the retail shelves.

  • KDA_3_10111 - --REPEAT: There were packages of animal crackers, beef jerky and coconut on the retail shelf that were chewed through by rodents. The license is voluntarily closed and no food is currently on sale.
  • KDA_6_20216 - --There are large holes in the seal surrounding the outdoor parameter of the facility allowing the entry of pests.
  • KDA_6_50111 - --There are large holes along the wall on the inside of the facility in the back storage area.
  • KDA_6_501111A - --There were hundreds of fresh rodent droppings throughout the store: on retail shelves, under retail shelves and in the back storage area.
  • KDA_6_50112A - --There are hundreds of fresh rodent droppings on the retail shelf, under the retail shelf and in the back storage area.

  • KDA_3_10111 - --There were boxes of bagged pretzels, bagged tortilla chips, coconut milk, Christmas candy, bagged popcorn, bagged cheeto puffs, seasonings, gummy candy, planters peanuts, kit kat candy and a whole rack of approximately 20 bags of bread in the back storage room that was adulterated by rodents. Packages/boxes were chewed though with rodent droppings and urine covering the packages. There were bags of coconut flakes on the retail shelf that were chewed through by rodents. COS- PIC voluntarily discarded all before mentioned products.
  • KDA_3_20215 - --There were approximately 6 cans of El Pato tomato sauce with large dents along the side compromising the seam. COS- PIC voluntarily discarded due to damaging.
  • KDA_6_20216 - --There are holes in the sealing element around the outside of the establishment large enough to allow the entry of pests.
  • KDA_6_50111 - --There are a few large holes in the wall next to the delivery doors in the storage area. There are large gaps between the walls and the concrete in the back storage area.
  • KDA_6_501111A - --There were two live mice, one inside a sticky trap, in the back storage area. There was also fresh rodent droppings estimated in the thousands throughout the facility (retail shelves, under the retail shelves and in the back storage area.
  • KDA_6_501111B4 - --There is a large amount of boxes piled up into big "mountains" in the back storage area with large amounts of nesting from rodents found throughout the piles.
  • KDA_6_50112A - --There are thousands of fresh rodent droppings on the ground in the back storage room, on and inside boxes in the back storage room, on the retail shelves throughout the facility and under the retail shelves throughout the facility.

Many boxes are chewed through, adulterated, and cannot be discarded due to no garbage space. The incoming food items and other food items present will be embargoed (#005080) as of today 12.3.15.
  • KDA_6_20215A3 - --There are 1/2 inch gaps under the entrance doorway, the back exit door and the delivery doorway in the back storage area large enough to allow the entry of pests.
  • KDA_6_50111 - --There is a large gap (1-2 inches) in the concrete seal along the outside of the facility (east side of building).
  • KDA_6_501111A - --There are extremely high amounts of fresh rodent dropping present throughout the retail shelving and in the back storage room (thousands). There were three mice found in the pest control sticky trap that were not discarded.
  • KDA_6_50112A - --There is fresh rodent droppings (thousands)on and under the retail shelves, on and under several boxes in the storage area and inside food packages throughout the facility.   • KDA_6_50111 - --There are large gaps (approximately 1 inch) between the wall and the concrete all along the storage area.
  • KDA_6_501111A - --There were approximately 8-10 live mice of various sizes in the back storage area.
  • KDA_6_501111B4 - --There were several (hundreds) boxes piled on top of each other in the back storage area leaving little to no walking space. Many boxes were chewed through with visible nesting inside and around the buried boxes.
