5-202.14 - --REPEAT: At the mop sink there is an atmospheric vacuum breaker installed. There is a green hose attached to the water outlet downstream of this vacuum breaker. The end of the hose is lying directly in the base of the mop sink below the flood rim. The vacuum breaker is leaking water from below the bell when it is turned on. Note - the manager said they have contacted a plumbed to have the vacuum breaker replaced since the previous inspection, but due to the holidays and the weather, the plumber has been unable to come out. KDA_4_60111A - --REPEAT: In the kitchen on a shelf of clean dishes above the 3 vat sink there is a plastic food container stacked with other clean dishes. This container has sticker residue remaining on the outside of the container. Corrected on Site (COS) - this container is being moved to dirty dishes to be cleaned. KDA_7_20111B - --There is a squirt bottle of hand sanitizer sitting directly on top of the paper towel dispenser at the hand sink just outside of the kitchen near the soda fountain. Corrected on Site (COS) - the manager moved this. There is a spray bottle of 409 and three bottles of ice machine cleaner being stored sitting directly on top of the ice machine in the ice machine room. 2-301.14(E) - --A gloved employee took dirty dishes to the 3 vat sink, rinsed them out, and put them with the other dirty dishes. She then grabbed clean gloves, took off her dirty gloves, put on the clean gloves, and cut open a bag of pizza sauce and began pouring it into the container for the make table. Corrected on Site (COS) - explained that she needed to wash her hands prior to handling clean gloves or food products. The employee washed her hands and changed gloves. 5-202.14 - --At the mop sink there is an atmospheric vacuum breaker (AVB) installed. There is a hose attached downstream of this AVB that is hanging down inside of the flood rim of the mop sink. When the water is turned on, water is leaking from below the bell of the AVB. Letting the manager know that the AVB needs to be replaced. KDA_3_30212 - --There are unlabeled squirt bottles with a yellow and red liquid inside at the sandwich make table that a kitchen employee verified were vinegar and oil. KDA_3_30412A - --There is a cup with no handle being used as a scoop in the cheese inside of the pizza make table. The cup is in direct contact with the cheese in the container. KDA_3_30511A1 - --There are loose tea bags being stored in the bottom of a milk crate in a cabinet below the self-service drink area. COS - the tea bags were put into their original protective box to be stored until they are used. KDA_3_30512 - --There is a jug of vinegar directly below the unshielded drain line of the sink near the sandwich make table. KDA_3_50118A2 - --In the cooler below the pizza make table there is an open container of ready-to-eat BBQ Beef that has no date on it. Upon confirming with an employee, this was identified as being opened Tuesday (12/15/15). COS - this was dated. KDA_3_60211B4 - --There are bags of ice in the walk-in-freezer that are for sale. These are labeled with information for Casey's in Ankeny, Iowa. These are bagged in this store, per the manager. Letting the manager know that the bags need to have their store number or other identifying information on them so that the bags indicate which Casey's store they come from. KDA_4_20211A2 - --There are two cracked plastic bowls on a shelf above the pizza make table. There is a rubber spatula that is chipping and fraying along the edge. COS - a kitchen employee discarded these. KDA_4_50111A - --The plastic lids/covers to the sandwich make table are cracking near the handles. KDA_4_50213B - --The tube from the half & half dispenser is cut straight, rather than on the diagonal, and sticking approximately two inches out of the dispenser. KDA_4_60111A - --There was a bowl on a shelf above the pizza make table. An employee in the kitchen said she believed all the dishes on this shelf were clean. The bowl has remnants of a barcode sticker remaining on the bottom of the bowl. COS - the bowl was moved to ware washing. There are single-use plastic cup lids in a cabinet below the self-service drink area on the retail floor. There is a cut out for the drain lines from the drink machines. These lids have fallen into this hole and are on the floor area. COS - these were discarded. KDA_4_60111C - --There is grease residue on the handles to the hot cases from the kitchen side. There is dust/dirt/debris buildup on the shelves below the hot cases in the kitchen. There is dust accumulation on the fan covers in the walk-in-cooler for the kitchen. KDA_4_90311C - --There are single-use to-go containers on a bottom shelf of the table opposite the sandwich make table, as well as a stack of single-use containers on a shelf near the walk-in-cooler. These containers are outside of their original protective packaging and are neither covered nor inverted. KDA_4_90312A - --There is a box of pan sheets for baking directly below the unshielded drain line of the sink near the sandwich make table. KDA_6_30111 - --There was no hand soap at the hand sink by the ice machine. KDA_6_30112 - --There were no paper towels at the hand sink in the kitchen upon beginning inspection. The manager said she is having trouble locating the key to refill the paper towel holder. COS - she put some paper towels on top of the paper towel dispenser upon request. KDA_7_10211 - --There is an unlabeled spray bottle with a pink liquid inside on a table of chemicals by the 3 vat sink. The kitchen employees are unsure what this is. COS - this was dumped out into the sink.
