- 5-202.14 - --At the mop sink, the faucet has a built in atmospheric vacuum breaker. The breaker has failed and leaks when the water is turned on. A hose is hooked to the faucet for filling mop buckets, and the end of the hose is hanging below the flood rim of the sink (removed hose) The hose must remain off of the faucet until the device is repaired or replaced.
- KDA_3_50118A2 - --In a reach in cooler at the coffee bar, one open jug of ready to eat commercially prepared milk not date marked, as per a waitress, it was opened two days ago (dated). At the salad bar, one container of ready to eat commercially prepared cottage cheese was not date marked, as per the waitress it was opened two days ago (dated).
- KDA_4_20211A2 - --In the kitchen, two plastic spatulas were broken and cracked. This can prevent proper cleaning, and pieces of plastic can end up in foods (discarded).
- KDA_4_30212B - --No food thermometer for taking temperatures of foods such as hamburgers. Thermometer obtained must have a thin probe.
- KDA_4_60111A - --A pitcher stored as clean at the coffee bar has a sticker and adhesive on the outside of the pitcher. In the kitchen, one plastic food container and six previously used single use plastic food containers stored as clean have stickers, remains of stickers, and adhesive on the outside of the containers (multi-use containers removed to 3-vat sink, and single use containers discarded). Soil and grime on the make table cutting board (removed for cleaning).
- KDA_4_60211E4 - --Mold is in the ice bin on the white plastic piece above the ice.
- KDA_4_70211 - --Wares not being sanitized in the dishwasher (no chlorine detected). The sanitizer bucket was empty and no new bucket was present. Instructed person in charge to wash and rinse in dishwasher, then sanitize wares in the 3-vat sink).

- KDA_6_30111 - --No soap at the men's restroom handsink (soap placed at sink).

- KDA_KSA65_689a - --This facility opened without a food establishment license one to one and one half months ago as per the person in charge. As per the KDA office, an application and $450.00 has been received. The facility is approximately 6156 square feet, which makes the amount owed $595.00. The owners owe $145.00 more, which must be submitted within 10 days to the KDA office.
