3-302.11(A)(2) - --In the walk in cooler, raw chicken was stored directly above raw ground beef patties. COS: Educated about the "silo" method and the kitchen manager rearranged the shelving unit. KDA_2_30114H - --2 employees on separate occasions donned gloves without washing their hands. Corrected on site (COS): Educated and employees washed their hands. KDA_3_30211A1a - --In the walk in cooler, raw shrimp and scallops were stored directly above raw beef and raw chicken. COS: Educated about the "silo" method/allergen and the kitchen manager rearranged the shelving unit. KDA_3_30211A1b - --In the walk in cooler, 2 flats of raw shell eggs were setting directly on top of containers of ready to eat diced mushrooms and onions. COS: Eggs were moved. KDA_3_30412B - --The scoop in the bulk rice and flour bins had the handle in direct contact with the food. KDA_3_30412F - --Both scoops for the steamed and fried rice were stored in water at 87.0F and 116.7F. KDA_3_30512 - --Four, 5 gallon buckets of duck sauce were stored in the employee restroom in the kitchen. COS: Food items were removed from the restroom. KDA_3_50118A2 - --In the sushi make table, cooked shrimp had no date marking. COS: Employee said it was opened 2 days ago and labeled the container 12/15. NOTE: In the walk in cooler several items were not dated. Shortly after the inspector arrived an employee went into the walk in cooler and started dating items. The inspector spoke with the managers about dating items when they are cooked/prepped. Inspector left the Listeria handout. KDA_4_10111A - --In the make table by the grill, several non-food grade white plastic containers with holes were being used to store food in direct contact with the container. In the walk in cooler, there were 2 non-food grade storage containers (1 clear/1 black) being used to store broccoli and cooked chicken in direct contact with the container. Setting on a cart by the waitress area in the kitchen, a red non-food grade container has fried wonton strips in direct contact with the container. Inspector let them know that they were non-food grade and that they need to be lined or transfer food into food grade containers. KDA_4_20211A2 - --The bulk kung pao sauce container lid has a hole cut out, but it is cracked and could have plastic fall into the sauce. The inspector suggests cutting the hole smooth. KDA_4_60111A - --The shelving units in the walk in cooler have a build up of food debris/soiled residue that could fall on or into food containers that are stored below when another container is moved. The shelving above the bulk flour/sugar containers has a build up of food debris/dust that could fall into the bulk bins that are below. The green wire shelving used for clean dishes has a build up of dust on the shelves. There is mold/mildew on the plastic beer lines in the beer walk in cooler. The metal wire shelving cart used to transport food in uncovered containers has a build up of food debris/soiled residue. KDA_4_60111C - --The bulk rice container has a build up of food debris on the sides and the lid. KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14. KDA_5_20515B - --The soda gun drain at the bar area is not draining properly and is starting to back up. The manager called Pepsi during the inspection to have them fix it. KDA_6_20211A - --In the walk in freezer by the kitchen, the light bulb is not shielded. KDA_6_50112A - --The main walk in cooler fan guards/ceiling have a build up of dust. The cold air return vent above the ice machine has a build up of dust. There is a build up of dust on the backside of the soda fountain, wall, and hoses in the kitchen waitress area. There is a build up of dust on the Pepsi soda syrup dispensing system by the beer walk in cooler. KDA_6_50118 - --The hand sink/paper towel dispenser by the warewasher/grill area has food debris/dust/dirt build up.
