KDA_3_50115B - --Ready to eat ground beef (113 F) was cooling in a deep pan with foil wrapped on the top of the pan in the walk in cooler. (COS- a chill bar was added to the container and the foil was loosened). KDA_4_50111A - --Paint was peeling on the ceiling in the walk in cooler. KDA_4_90311A - --A case of clam shell containers were stored on the floor north of the dishmachine. (COS-removed from floor). KDA_5_40312 - --Condensation from the walk in freezer was dripping into a bucket in the east storage room. KDA_7_10211 - --There was an unlabeled white bottle setting on the ledge of the mop sink. The employee did not know what was in the container. "Corrected on site" (COS-container was discarded).
