- KDA_3_50118A2 - --In the RIC in wait station a gallon jug of milk was not date marked. Server said she opened it on Sunday 10/11. COS, dated correctly.
- KDA_3_60311B - --The menu does not have the disclosure for made to order eggs or steak.
- KDA_4_10111A - --In the Maximum RIC cheese slices were stored in direct contact with a non-food grade Sterilite container. COS, owner moved to a food grade container. In the kitchen raw potatoes were stored in a non-food grade container. COS, removed.
- KDA_4_20211A2 - --Below the steam table a spatula was stored as clean with a melted handle making it difficult to clean. In a crock by the stove a spatula was stored as clean with a chipped edge. COS, both items discarded.
- KDA_4_60111A - --On the shelves in the NE corner of the kitchen food storage containers stored as clean had stickers left on them. COS, moved to be cleaned and sanitized.
- KDA_6_20211A - --The lights in the kitchen over the butcher block table are not shielded.
- KDA_6_20214 - --The doors to the men's and women's restroom do not have self closures.
- KDA_6_20215A3 - --In the trailer the door on the west end is not sealed at the top leaving a 1" or larger gap allowing pests to enter.
- KDA_6_50112A - --There are old rodent droppings on the shelf below the 3-vat sink and on the floor in the back storage room below the shelves.
- KDA_7_10211 - --In the kitchen 2 spray bottles of purple liquid was not labeled. The owner said it was degreaser. COS, labeled.
